Abstract (EN)
This study in patients diagnosed with Systematic Lupus Eritematozis disease clinics, radiological findings showing the involvement of the brain with neuro-psychiatric not detect brain involvement (MRI) among patients with different brain perfusion changes in the localization of whether and perfusion MRI rCBV, rCBF, MTT and TTP was aimed to evaluate the parameters.The study of 21 women and 3 men age 19-56 were used in a total of 24 cases. Cases whole-body imager 1.5T MRI apparatus (Gyroscan Intera Master, Philips) using a standard head coil was shooting. All patients TSE (turbo spin echo) sequence with axial T2-weighted, FLAIR and T1-weighted conventional examination and gradient echo (FFE) sequence (TE: 30, TR: 500) with dynamic perfusion studies were performed. And 50-slice and 16 slice thickness 7 mm in the form of dynamic cross-section, respectively. Injector pump using 18 G IV catheter with 0.2 mmol / kg contrast agent (gadolinium) was given.7 sec delay time and cross-sections were obtained. The most frequently reported in the literature, the affected areas by taking into consideration the average ROI (region of interest) by setting the width, frontal white subtance (80-120 mm2), temporal lobe whit substance (70-90 mm2), occipital white subtance (70-100 mm2) Caudat nucleus (20 -30) mm 2, lentiform nucleus (140-200 mm2), pons (100-130 mm2), cerebellum (120-160 mm2), internal capsule posterior leg (60-80 mm2) measured by the localization of the left half of the brain perfusion maps and rCBV, MTT, rCBF, TTP values were obtained.24 patients from 14 neuro-psychiatric involvement and SLE patients (12K, 2E, 36.7 ± 11.8 years), 10 neuro-psychiatric involvement of non-SLE (9K, 1E, 38.4 ± 10.8yıl) and 15 healthy controls (10K, 5E, 35.1 ± 10.5 years) was performed on the group. Brain MRI of 24 patients taken in 9 (37.5%) had abnormal MRI features. NPSL (+) at a rate of 42.9% in patients with abnormal MRI findings are being monitored NPSL (-) by 30% in patients with abnormal MRI findings were determined. White matter lesions were most frequent abnormality and it was followed by infarcts. 4 patients had multiple MRI lesions.Between groups in different regions of the brain perfusion MRI rCBV, rCBF values of MTT and TTP were compared statistically. Diagnosis between the groups in frontal lobe white matter, no significant difference between CBF (p = 0.045), temporal lobe white matter, a significant difference between the CBF (p = 0.024), occipital lobe white matter between MTT and CBF significant difference (p = 0.022, p = 0.003), caudate nucleus between the MTT and CBF significant difference (p = 0.045, p = 0.031), nucleus Lentiform between MTT and CBF significant difference (p = 0.025, p = 0.005), Ponsda difference between MTT and CBF (p = 0, 02, p = 0.004), MTT in white matter Serebeller significant (p = 0.048), internal capsule in the posterior leg MTT significant (p = 0.007) differences were detected. Frontal lobe white matter in patients with CBF reduction in hypo-perfusion, temporal lobe white matter CBF reduction in the hypo-perfusion, occipital lobe white matter hypo-perfusion elongation at MTT, MTT prolongation Lentiform nucleus and hypo-perfusion decreased CBF, and CBF Pons?ta prolonged MTT reduction hypo-perfusion, Serebeller MTT prolongation in the white matter and internal capsule posterior hypo-perfusion in leg elongation MTT showed hypo-perfusion.As a result of lupus patients compared with the control group, multiple regions of the brain in particular, temporal, frontal and parietal lobe perfusion MRI revealed hypo-perfusion areas. Especially in patients with involvement NPSLE in different regions of the brain, perfusion MR images obtained from the quantitative rCBV, rCBF, MTT and TTP volumes are assessed and provides useful clinical information. SSS symptoms in patients with SLE correlated with MRI findings. Suspected of involvement in patients with high probability NPSL with MRI perfusion MRI to provide information wounded am convinced the combination.Key words: MRI, Perfusion, SLE, SSS involvement
