Evaluation of the relationship between serum visfatinlevel and coronary artery collateral development

Abstract (EN)
Patients with well-developed coronary collateral circulation have a better prognosis. Angiogenesis and arteriogenesis play a role in the development of coronary collateral arteries. In prior studies, visfatin has been shown to increase angiogenesis. However, there is lacking information regarding its relation with coronary collateral development. In this study, we aimed to examine the relationship between serum visfatin level and coronary collateral circulation in patients with coronary artery disease. A total of 84 patients with a ≥ %90 stenosis of proximal or middle segments in at least one of the three main epicardial coronary arteries who underwent coronary angiography with a pre-diagnosis of stable coronary artery disease were included in our study. Collateral development was assessed in four stages (0-3) according to Rentrop-Cohen classification. Serum visfatin levels were measured by sandwich ELISA (enzyme-linked immunosorbent assay) method. Serum visfatin level was higher in the good collateral group than in the bad collateral group (good collateral 8.89 ± 8.30 ng / ml, bad collateral 3.43 ± 2.87 ng / ml p = <0.001). Correlation analysis revealed a moderate statistically significant positive correlation between collateral grade and serum visfatin levels (rs = 0.441, p <0.001). Multivariate regression analysis revealed that the absence of DM, the presence of total occlusion and high serum visfatin levels were found to be independent predictors of good coronary collateral artery development. For the value of 2.75 ng / ml visfatin in predicting the development of good collateral in the ROC curve is sensitivity and specifity of 72.5% and 54.5%, respectively. In addition, serum visfatin level was found to be statistically significantly higher in patients with multi-vessel disease, total occlusion or severe lesions in right coronary artery. Our results show that high serum visfatin level is an independent predictor in good coronary collateral development.
