Examination of the relationship of CRP/albumin, platelet/lymphocyte, neutrophil/lymphocyte, lymphocyte/monocyte and monocyte/hdl values with clinical parameters in schizophrenia patients

Abstract (EN)

Schizophrenia is a systemic degenerative mental disorder whose etiologic factors vary. There are studies which show that systemic inflammation response in patients diagnosed with schizophrenia is different from normal and abnormal. According to findings of previous studies, it is understood that, systemic inflammation measures such as IL1 and CRP are found higher in schizophrenia patients compared to healthy individuals The aim of this study is comparison of CRP/Albumin, Platelet/Lymphocyte, Neutrophil/Lymphocyte, Lymphocyte/Monocyte and Monocyte/HDL values of patients diagnosed with schizophrenia with healthy control groups and investigation of relationship between these values and age of onset, duration of ilnesses, number of hospitalizations, number of episodes, mean antipsychotic dose (olanzapine equivalent dose), family history (history of psychiatric illness in 1st and 2nd degree relatives) in schizophrenia patients. Patients who were hospitalized in the psychiatry service of Adıyaman University Training and Research Hospital between 2020 and 2022 with a diagnosis of schizophrenia and healthy individuals who applied to the psychiatry outpatient clinic for reasons such as employment etc. and who did not have any psychiatric diagnosis in the same time period were included in the study. Between specified dates, patients, whose age between 18-65, who is diagnosed with schizophrenia according to DSM-5 diagnostic criteria and had no chronic diseases were included in the study. Last hospitalization datas of patients, whose hospitalization is more than one, were taken as reference. 103 patients with schizophrenia and 112 healthy controls were included to the study. In the selection of both groups, we paid attention not to have statically significant difference in terms of gender and age. CRP/Albumin, Platelet/Lymphocyte, Neutrophil/Lymphocyte, Lymphocyte/Monocyte and Monocyte/HDL values of patients diagnosed with schizophrenia and healthy controls were analyzed retrospectively. In addition, the relationship between these inflammatory parameters, age at onset of illness, duration of illness, number of hospitalizations, number of episodes, mean antipsychotic dose (olanzapine equivalent dose), family history (history of psychiatric illness in 1st and 2nd degree relatives) were investigated. IBM SPPS (Statistical Pack age for the Social Sciences) version 26.0 program was used for statistical analysis. It was determined by Kolmogorov-Smirnov and Shapiro-Wilk tests if the datas were normally distributed. For comparison between groups, independent sample t test was used for normally distributed data and Mann-Whitney U test was used for non-normally distributed data. One-way ANOVA analysis was used for multiple groups. Statistical significance level was accepted as p<0.05 for all values. According to the statistical analysis; NLR, PLR, CAR values were found to be statistically significantly higher in schizophrenia patients compared to healthy control group (p<0.001). Statically, LMR value in control group was found significantly higher compared to the schizophrenia patient group (p=0.001). Spermann Correlation test was used to determine whether there was a relationship between age, CAR, PLR, NLR, LMR, MHR, duration of illness, number of hospitalizations and olanzapine equivalent dose in schizophrenia patients. According to the analysis, a positive correlation was found between age and CAR, PLR and NLR values (respectively p=0,357 p=245 p=0,213). In addition, a positive correlation was found between duration of illness and number of hospitalizations and CAR, PLR and NLR, and a negative correlation was found between duration of illness and LMR. There was no correlation between olanzapine equivalent dose and inflammatory parameters. According to the binary logistic regression model, when the effect of being diagnosed with schizophrenia or being in the control group was analyzed, it was found that CAR and NLR were statistically significantly associated with schizophrenia (p=0.04, p=0.000). In our study, which was based on patients diagnosed with schizophrenia and control group subjects, among the inflammatory parameters we determined, especially CAR, PLR and NLR levels were found to be significantly higher in patients diagnosed with schizophrenia. This conclusion suggests that especially schizophrenia disease has a relationship with inflammation and inflammatory process.
