Abstract (EN)

Comparison of Patients with Parkinson's Disease and Parkinson's Disease who Treated with Deep Brain Stimulation to the Subtalamic Nucleus in Terms of Cognitive Function, Emotional Status, Quality of Life, Disease Severity, Character and Temperament, Neuropsychiatric Symptoms and Executive Functions Objective: The aim of this study was to compare Parkinson's disease and Parkinson's disease who treated with Deep Brain Stimulation to the Subthalamic Nucleus in terms of disease severity, executive functions, emotional status, cognitive functions, quality of life and temperament characteristics and to examine which of these factors are more closely related. Materials and Methods: A control group consisting of 15 Parkinson's disease patients who treated with Deep Brain Stimulation to the Subthalamic Nucleus and 15 Parkinson's disease patients matched for age, gender and education were included in the study. The patient groups were first given a sociodemographic data form, then the Hospital Anxiety and Depression Scale, Temperement and Character Envantory and The Parkinson's Disease Questionnaire. Afterwards, Mini Mental Test, Neuropsychiatric Inventory, Stroop test, Unified Parkinson's Disease Rating Scale, HoehnYahr Scale, Verbal Fluency Test were applied to all groups. Results: Total values of letter K, letter A, letter S and fruit and market total values in verbal fluency test, time orientation and total orientation values in mini mental test, excitement to Novelty Seeking Cooperativeness 1 (NS1), empathy (C2), helpfulness (C3) subcategory scores and depression, anxiety, apathy and irritability values in the Neuropsychiatric İnventory were found to be higher than the Parkinson's patient group.The anxiety scores of Parkinson disease patients on the Hospital Anxiety Depression scale, the total values on the UPDRS and the values of all sub-categories of this test were higher compared to the Parkinson group that treated with Deep Brain Stimulation.There was no significant difference between the two groups in Stroop test scores and Parkinson Disease Questionnaire. As a result of the correlation analysis, it was found that the performance in the verbal fluency test worsened as the anxiety and depression levels in Parkinson's disease patients increased; As the Mini Mental Test scores increased, the performance in some sub-categories of verbal fluency test performance improved; as UPDRS motor examination scores increase, category fluency test fruit total, market total, animal repetition values decrease, as Hoehn Yahr Stating Scores increase, verbal fluency test performance worsens, as disease severity increases in UPDRS test, Stroop test performance and mini mental test scores decrease; It has been shown that cognitive functions worsen as depression and anxiety levels increase. Mini Mental Scores and Stroop test scores of Parkinson's disease group; no significant correlation was found between depression and anxiety scores and Mini Mental test scores. In the results of correlation analysis in Parkinson's disease who treated with Deep Brain Stimulation, there was no relationship between anxiety and depression leves and verbal fluency test; there was no relationship between the mini mental test and the verbal fluency test; As Mini Mental language scores increase, animal repetition values decrease in category fluency test, as Mini Mental orientation total scores increase, animal and market repetition values increase in category fluency test. As Mini Mental language scores increase, animal repetition, fruit total, fruit repetition values in category fluency test also increase. It has been shown that as the UPDRS moto examination scores increase, the category fluecy test animal total values decrease. As the memory, attention and calculation scores of Parkinson's disease who treated with Deep Brain Stimulation increase, S4 correction test scores decreased, as the recall scores increased, s1, s2, s3, s4 time and s2, s4 error scores in the Stroop test decreased. As Mini mental total scores increase, s1, s2, in the stroop test, s3, s4, time scores decreased; ıt was determined that Mini mental test scores decreased as UPDRS scores increased. Conclusion: This cross-sectional, prospective and controlled study, Parkinson's disease and Parkinson's Disease who treated with Deep Brain Stimulation to the Subtalamic Nucleus applied cognitive function, emotional status, character and temperament characteristics, disease severity and quality of life were compared in terms of executive fuctions and associated factors were investigated, where the English and Turkish liteature as we can see in the first study. Our findings of the increase in disease in Parkinson's disease adds cognitive and executive functions; reducing the severity of Parkinson's disease and motor functions of the DBS treatment and executive functions, emotional status and has shown to improve cognitive function. Our findings expand the existing data showing the motor symptoms and neuropsychiatric effects of Deep Brain Stimulation, which is crucial fort he development of strategies that could potentially reduce motor findings and neuropsychiatric symptoms in Parkinson's disease in the Turkish population.
