Clinical outcomes of the use of large size acetabular cup in the treatment of unilateral Crowe type 2 and Crowe type 3 developmental dysplasia of the hip

Abstract (EN)
Objective : Developmental dysplasia of the hip is one of the most important causes of hip joint osteoarthritis that occurs at an advanced age. Total hip arthroplasty techniques differ among surgeons in this patient group. In this study, the early clinical and functional results of the use of large size acetabular cups in total hip arthroplasty performed in the patient group of the 2nd and 3rd type according to the Crowe classification were investigated. Materials and methods : Patients that underwent total hip arthroplasty by a single surgeon for hip joint osteoarthritis in our hospital between 2010 and 2018 on the basis of developmental dysplasia of the hip were retrospectively scanned. 102 patients with unilateral Crowe type 2 and Crowe type 3 dysplasia and recorded radiological images were included in the study. Results : 60 of 102 patients included in the study had Crowe type 2 dysplasia, while 42 had Crowe type 3 dysplasia. Harris hip score of the patient group with Crowe type 2 dysplasia was determined preoperatively as (35.57 ± 6.03) and postoperatively as (88.50 ± 4.17); while Oxford hip score was found as preoperative (20.45 ± 2.93) and postoperatively as (42.73 ± 2.91). Harris hip score of the patient group with Crowe type 3 dysplasia was determined preoperatively as (33.43 ± 6.17) and postoperatively as (87.60 ± 3.94); while Oxford hip score was found as preoperative (20.57 ± 3.04) postoperatively as (41.50 ± 3.19). In 62 both patient groups results had a statistically significant increase. In the postoperative period, HOOS-PS scores were found as 89.66 ± 4.76 for the Crowe type 2 dysplasia patient group and 88.40 ± 5.31 for the Crowe type 3 dysplasia patient group. No statistically significant difference was found between the groups in the results of Harris hip scoring, Oxford hip scoring, and HOOS-PS scoring by the grouping made according to the postoperative hip joint rotation center elevation degree (<15 mm, 15-20 mm,> 20 mm). Conclusion : In patients with unilateral Crowe type 2 and Crowe type 3 dysplasia, good results were obtained in terms of implant survival, patient satisfaction and physical function in the early postoperative period with the use of large size acetabular cups. The amount of hip joint rotation center elevation had no effect on the results. Keywords : Arthroplasty, dysplasia, hip, acetabular cup, rotation center
