The relationship between microalbuminaria and neutrophil/lymphocyte in patients with type 2 diabetes mellitus

Abstract (EN)
Objective: Diabetes mellitus (DM) is a chronic and broad-spectrum metabolic disorder characterized by hyperglycemia, which causes multisystem involvement by affecting many organs, resulting from relative or absolute insulin deficiency or 'insulin resistance' in peripheral tissues against the action of insulin. Diabetic nephropathy is a progressive kidney disease caused by glucose homeostasis disorder that progresses with glomerular and tubular, structural and functional changes. Chronic inflammation has an important role in the formation and development of type 2 diabetes. Urine albumin levels (albumin/creatinine ratio, amount of microalbumin in 24-hour urine) are classically used to determine the severity of diabetic nephropathy. Neutrophil-lymphocyte ratio is also an inexpensive and easy indicator of subclinical inflammation. In this study, we aimed to investigate the relationship between microalbuminuria level and Neutrophil/Lymphocyte ratio in patients with Type 2 DM. Materials and Methods: This study was created by the retrospective evaluation of Type 2 DM patients, 57 males and 70 females, aged between 29-79 years, who admited to Bezmialem Vakıf University Medical Faculty Hospital between November 2011 and November 2020, without any kidney disease. . The amount of microalbumin and NLR (neutrophil lymphocyte ratio), A1c, LDL, TG, creatinine, sedim, crp, ferritin were evaluated in the 24-hour urine during the examination. Results: There was no significant difference between the groups in the study in terms of mean age, gender, creatinine, TG, LDL, AST, ALT, and CRP. The neutrophil lymphocyte ratio of the normoalbuminuric group was 1.68 (0.7-10.2), and the neutrophil lymphocyte ratio of the microalbuminuric group was 2.15 (0.98-18.14). The difference between the groups was found to be statistically significant. (p<0.001). Conclusion: A significant correlation was found between microalbuminuric patients neutrophil lymphocyte ratio and normoalbuminuric patients neutrophil lymphocyte ratio. There was no significant relationship between total albumin amount in urine and neutrophil lymphocyte ratio. Neutrophil-lymphocyte ratio may be a marker of early stages of diabetic nephropathy.
