Assessment of patients' ecpectations with total hip prosthesis in Turkish Population

ASSESSMENT OF PATİENTS? EXPECTATİONS WİTH TOTAL HİP PROSTHESİS IN TURKISH POPULATIONObjective: Patient expectations from orthopedic surgeries may differ according to age, gender, diagnosis, life style and cultural habits. Therefore, knowledge related to life style and cultural habits are required. The objective of this study was to determine and evaluate the expectations of patients indicated total hip arthroplasty (THA) in Turkish society.Method: Primary total hip arthroplasty patients who had undergone surgery at least 6 months before with 18 and more age were enrolled in the study. All the patients were operated by the same surgeon. In total, 94 patients (35 male, 59 female) were included with mean age 57.1±14.6 years. Patients were evaluated with total hip arthroplasty clinical assessment questionnaire, a 25-item expectation questionnaire, Harris Hip Score, sit-to-stand test, up and go test, timed up and go test, stair up and down tests and SF-36 Quality of Life QuestionnaireResults: Mean Right Hip Harris Score of the patients was 92.7± 1.6 and mean Left Hip Harris Score was 93.2± 8.8. SF-36 Physical Function mean total score was 69.6 ± 16.4, SF-36 Physical Health mean total score was 65,6 ± 42,0, SF-36 Emotional Problems mean total score was 72.0 ± 44.7, SF-36 Energy/Fatigue mean total score 66.2 ± 23.3, SF-36 Emotional Health mean total score was 78.3 ± 17.4, SF-36 Social Function mean total score was 84.6 ± 20.2, SF-36 Pain mean total score was 74.0 ± 28.4, SF-36 General Health mean total score was 70.7 ± 23.1. Mean sit-to-stand test result was 20.6 ± 7.5 sec, mean up and go test result was 7.6 ± 3.2 sec, mean timed up and go test result was 4.5± 0.7 sec and mean stair up and down test result was 14.8± 9.1 sec.Improvement of walking ability was the most important expectation of the patients, while the possibility of working again was the least important. Pain relieve due to the operation was the most satisfied parameter whereas activity level during the last 3 months was the least satisfied.Conclusion: Determining patient expectations and recovery rates help to specify realizable and feasible expectations. Moreover, restricted and difficult activities could be determined through the responses of an expectation questionnaire and this might enable to plan problem-specific treatment programs. The most important expectations of Turkish patients in our study were functional restoration and pain relieve. Activity, work levels and stair climbing abilities were the least satisfied parameters. We consider that the information obtained from our study may help to determine the treatment modalities and treatment goals.Key Words: Total hip prosthesis, Turkish Population, expectation
