To evaluate of effect for examination participation fee?s applications to the health service appeals and health expenses in Turkey

Abstract (EN)
It has been targeted for evaluation of effects to the health expenses and health service appeals of the applying of exam participation fee in Turkey, in our study. According to the health facilities in Turkey, it has been tried to fix the effects on pharmaceutical expenses, quantities of prescription health expenses and service request of applying of outpatient therapy and examinations of dentists? participation fee that has been applied as different rates between the years of 2008-2011. The data of the study has been taken from SSI and MOH. The scope of the study has been generated to appeal to the pharmacies, private hospitals, governmental hospitals and university hospitals all over Turkey between the years of 2008-2011. It has been seen that total patients appeals has increased between 2008-2011 and this raise in the year of 2011 has been shown the most in university health facilities and private health facilities, the raise in the year of 2010 at the private health facilities and third stage governmental health facilities is lesser then the raise in the year of 2009, the minimum raise in the year of 2010 happened at the second stage private health facilities.As parallel of patients? appeals raise, the invoices have increased and mostly in third stage governmental health facilities and the private hospitals. It has been shown that the numbers of appeals of the outpatient increased between the years of 2008-2011, but the raises between the years of 2010-2011 less than the year of 2009. When the difference of total appeals in detail is observed, 2010 and 2011 have been shown the serious difference, and the patient's appeals in 2010 and 2011 have been detected to be higher than the other years. When the difference between numbers of appeals and the amount of invoices is examined in detail regards to the years, it has been fixed that the years of 2009 - 2011 shows the serious difference against to the other years and the amount of the other patients' invoices are higher than the other years and this raise is significant as statistical. When the rate which the appeals according to the years to SSI's contain, it determines in detail, the others has been shown the difference except the rate of inpatient appeals to the SSI's content. When the evaluation has been done deal with the rate of total appeals to the SSI content, it has been noticed that the rate increased as seriously in 2008-2011 and the difference of raise is significant as statistical (F=78,019; p=0,000).It has been seen that the numbers of prescription increased between the years of 2008-2011. The raise on the quantities of prescription have been reflected to the costs, but the costs have been decreased in 2010. It has been seen that the quantity of outpatient's prescription against to the per hundred appeals are decreasing. While between the years of 2008-2010 the most prescription has been prescribed in the first stage health facilities of MOH, the raising in second stage health facilities has been determined. Additionally the numbers of prescription has been shown serious rising by the business facilities doctors starting from 2010. When the total numbers of prescriptions have been evaluated, a serious decreasing has been detected in the year of 2011. The numbers of prescription are increased in the governmental institutions between the years of 2008-2011, but decreased in the other administrations. According to the changing dates of applying for outpatient therapy participation fee, when the quantities and amounts of prescription reviews, it has been detected the numbers of prescription between October of 2008 and June of 2009 are higher than the adequate years. The same result has been reached deal with the amount of prescription cost. When the difference in the numbers and cost amount of prescription in the therapy of outpatient and inpatient has been looked at, it has been determined that there is significant difference as statistical (t=35,926; p=0,000) and the cost amount and quantities of prescription are higher in outpatient therapy. It has been detected that the quantity of prescription (t=25,639; p=0,000) and cost amount of prescription (t=13,217; p=0,000) are significant different as statistical on the treatment of routine and reported pharmaceuticals, at the routine therapy the quantities of prescription are higher, but at the reported treatment the cost amounts of prescription are most higher.The truth of our country's population which raising getting elder will be appeared our expenses of health growing up. It is another factor that we have been met that the expenses of health will increase because all the citizens have been put to the social security by General Health Insurance. The health authorities should some regulations like the world authorities made to be enough to the population that are growing up and for the scarce source of our country. Thus the expense of health has been tried under control by some regulations between the years of 2008-2011, but has not been enough. As the results by our study has been shown that the participation fee is not enough, our country needs complementary health insurance and some policies to manage offers should be in new regulations.
