Abstract (EN)

Vestibular migraine is a balance system disorder seen in individuals with a history of migraines. The sensation of dizziness can occur alone or along with other migraine symptoms such as severe headache and nausea. There is no standard approach to the treatment of vestibular migraine; the primary aim is to reduce the frequency of migraine attacks. For this purpose, both systemic and local treatments are applied. Botulinum toxin application and acupuncture are among the locally approved treatment approaches. The aim of this study is to investigate the effect of botulinum toxin applied to acupuncture points on vestibular symptoms, headaches, depression, and cutaneous symptoms in patients with vestibular migraine. Patients diagnosed with vestibular migraine or probable vestibular migraine, based on the criteria specified by the BARANY society, were included in the study. Patients were grouped based on whether or not pathology was detected in their vestibular test panel results. Participants completed the Migraine Disability Assessment Scale (MIDAS), the Dizziness Handicap Inventory-Screening Form (DHI-S), the Vertigo Symptom Scale-Short Form (VSS-SF), the Depression Anxiety Stress Scale (DASS-21), and the Allodynia Symptom Checklist (ASKL) forms. In the application, a total of 50 units of Onabotulinum toxin A were injected into acupuncture points for each patient. Patients who received the botulinum toxin were re-evaluated in the first, third, and sixth months. In both the third and sixth month results, there was a statistically significant difference in MIDAS, VSS-SF, DHI-S, DASS-21 and ASKL values compared to baseline values. It was concluded that pathological vestibular test results, along with migraine vestibulopathy did not change the improvement after botulinum toxin application and did not create a statistically significant difference.
