Retrospective evaluation of the effects of different anesthesia methods on awakening agitation in patients understanding vitroreoretinal surgery

Abstract (EN)

Purpose: Agitation seen during recovery from general anesthesia is an important postoperative problem. In our study, it was aimed to compare the effects of different anesthesia methods on awakening agitation by retrospectively examining the files of patients who underwent vitreoretinal surgery. Materials and Method: The study was carried out by retrospectively examining the files of a total of 100 patients aged between 18-70 years who underwent vitreoretinal surgery, included in the American Society of Anesthesiologists (ASA) I-III classification. The patients were divided into two groups as Group T (Total intravenous anesthesia) and Group D (Desflurane) according to anesthesia maintenance. Standard monitoring values (systolic and diastolic blood pressures, heart rates, mean arterial pressure, peripheral oxygen saturation, electrocardiogram, end-tidal carbondioxide), 5-point extubation quality scores and waking moment (0.minute (min)) recorded in the anesthesia follow-up form of the patients who were taken to the operating room. Richmond agitation sedation scale (RASS) values, monitorization values in the recovery unit (15. min, 30. min), RASS values, presence of nausea and vomiting, and additional analgesic requirements were examined. Findings: Between D and T groups; There was no statistically significant difference between age, gender, mean body mass index, ASA classification, and mean duration of surgery and anesthesia (p>0.05). Between D and T groups; When hemodynamic (heart rate, systolic, mean and diastolic arterial pressures, end-tidal carbondioxide values and peripheral oxygen saturation) parameters were compared, no statistically significant difference was found between the groups (p>0.05). When the extubation quality levels of the patients were compared, it was seen that they were better in the TIVA group. (p<0,001). When the waking agitation levels of the patients were compared, it was seen that the 0 minute agitation levels between the groups were lower in the TIVA group. (p<0,001). When the additional analgesic requirements of the patients were compared, it was seen that the additional analgesic requirement was less in the TIVA group. (p=0,005). Conclusion: In our study, it was observed that the choice of total intravenous anesthesia as the anesthesia method in patients who underwent vitreoretinal surgery decreased the awakening agitation compared to those who underwent inhalation anesthesia.
