Abstract (EN)
Neutropenic fever is more common in cancer patients receiving chemotherapy treatment. It is a clinical condition that may result in serious infections, may require long-term treatment, and has a high mortality. The first place of application for the patient with neutropenic fever is the emergency services. For this reason, risky patients who need early diagnosis and prompt treatment should be identified in emergency departments. For this purpose, various scoring systems have been developed. These are febrile neutropenia specific MASCC and CISNE scores. The patients included in the study; Patients who applied to Dicle University Medical Faculty Hospital Adult Emergency Service between 01.01.2018 and 31.12.2021 consisted of patients. The study was done retrospectively. The data obtained in the study were evaluated using the SPSS (Statistical Package for the Social Sciences) 21st package program and a p value of < 0.05 was considered statistically significant. . Univariate statistical analyzes were performed using the Man Witney U Test for categorical variables. A p value of < 0.05 was considered statistically significant. The cut-off points of MASCC,CISNE,NEWS,MEWS and qSOFA and their sensitivity and specificity were calculated by ROC curve analysis. When we look at the early warning scores in our study in terms of mortality, MASCC, CISNE, MEWS, NEWS and qSOFA were found to be significant in determining morality. When we performed the ROC curve analysis, NEWS with the highest AUC value was seen among NPA patients. Then, the AUC value was ranked as MEWS, qSOFA, MASCC and CISNE. In addition, biomarkers such as PCT and Lactate, which we examined in our study, were also successful in predicting mortality. As a result, our study was concluded in accordance with its purpose. Since febrile neutropenia is a serious clinical condition that creates both mortality and economic burden, our study should be supported by other studies.
