Abstract (EN)

Ryretrospective Evaluation Of Electrocardiography (Ecg) And Laboratory Parameters In Patients Diagnosed With Urosepsis Admitted to The Emergency Department Objective: This study aimed to investigate the prognostic value of the frontal QRS-T angle and inflammatory markers in evaluating complicated urinary system infection (UTI) and urosepsis cases, as well as their association with mortality. Methods: This retrospective, randomized, observational, single-center study was conducted at the Emergency Medicine Department of Süleyman Demirel University Faculty of Medicine Hospital. Patient files diagnosed with urinary tract infections in the emergency department between 15/09/2022 and 01/10/2024 were reviewed, and those with complete records were included in the study. Data on age, gender, vital signs, hemogram, biochemical parameters, CRP, PCT, and lactate levels were recorded. ECG findings were also reported. Results: A total of 249 patients diagnosed with urinary tract infection were included. Of these, 42.6% (n=106) were male and 57.4% (n=143) were female. When evaluated by subgroups, 20.5% (n=51) had simple urinary tract infections, 25.3% (n=63) had complicated urinary tract infections, and 54.2% (n=135) had urosepsis. In our study, low albumin and calcium levels, as well as elevated inflammatory indices, were found to be significantly associated with both subgroup classification and mortality. The cut-off value for the frontal QRS-T angle was determined as 48, and a significant increase in mortality was observed in cases above this threshold (p<0.001). In Cox regression analysis, the FQRS-T angle and albumin levels were found to be independent predictors of mortality. Conclusion: In our study, the mean frontal QRS-T angle was 51.75±41.40 in surviving patients and 88.73±58.16 in deceased patients. The increase in the frontal QRS-T angle is noteworthy. Our findings suggest that the FQRS-T angle is a valuable electrocardiographic parameter that can be used for early risk prediction in the management of urosepsis.
