Abstract (EN)
Introduction and Objective: This retrospective study retrospectively evaluated data from a total of 114 patients, 60 of whom were male and 54 of whom were female, who were admitted to the Pediatrics and Pediatric Diseases Service of Adiyaman Education and Research Hospital and received red cell suspension transfusion due to symptomatic anemia or active bleeding. Material and Method: This retrospective study retrospectively evaluated data from a total of 114 patients, 60 of whom were male and 54 of whom were female, who were admitted to the Pediatrics and Pediatric Diseases Service of Adiyaman Education and Research Hospital and received red cell suspension transfusion due to symptomatic anemia or active bleeding. Results: This appears to be a study report on the effects of erythrocyte (red blood cell) transfusions on patients. The study included 114 patients, with 60 in Group 1 and 54 in Group 2. The patients' blood groups were distributed as follows: 43 (37.7%) O Rh+, 33 (28.9%) A Rh+, 21 (18.4%) B Rh+, 6 (5.3%) AB Rh+, 4 (3.5%) A Rh-, 2 (1.8%) B Rh-, and 1 (0.9%) AB Rh-. The average age of the patients was 73.33 ± 69.35 months (range: 2-226 months), and their average weight was 19.75 ± 15.32 kg (range: 3.7-56 kg). The average length of hospital stay for the patients was 3.02 ± 4.57 days (range: 1-21 days). When the indications for transfusion were analyzed by patient group, it was found that 71.7% of the Group 1 patients received transfusions for anemia, 10% for bleeding, 8.3% for malignancy, 6.7% for other reasons, and 3.3% for pre-op purposes. For Group 2 patients, the corresponding figures were 68.5% for anemia, 24.1% for bleeding, 1.9% for malignancy, 1.9% for other reasons, and 3.7% for pre-op purposes. When the pre- and post-transfusion full blood count results of the 114 transfused patients were evaluated, there were statistically significant differences in Hgb (p=0.001), Hct (p=0.001), RDW (p=0.001), MCH (p=0.001), MCHC (p=0.001), and MCV (p=0.001), but no statistically significant differences in WBC (p=0.509) and RBC (p=0.693). When the pre-transfusion full blood count results of the transfused patients were compared between groups, there was a statistically significant difference in the mean values of WBC (p=0.038) and LYM (p=0.008). There were no statistically significant differences in the mean values of hemoglobin (p=0.544), hematocrit (p=0.757), PLT (p=0.675), MCH (p=0.420), MPV (p=0.610), and MCHC (p=0.827) between the groups. When the post-transfusion full blood count results of the transfused patients were compared between groups, there was a statistically significant difference in the mean values of WBC (p=0.024), BASO (p=0.002), and LYM (p=0.029). There were no statistically significant differences in the mean values of hemoglobin (p=0.959), hematocrit (p=0.835), PLT (p=0.566), MCH (p=0.069), MPV (p=0.216), and MCHC (p=0.580) between the groups. Conclusion: Blood and blood products transfusion as a clinical topic is rarely discussed. Regardless of the indication, the expected clinical improvement before transfusion, alternative options to transfusion, and the benefit-risk analysis of transfusion should be evaluated before transfusion is performed. Transfusion criteria may vary from center to center. Clinicians at the center where the study was conducted follow current literature and consider hemodynamic and blood parameters of critically ill patients in the decision-making process for blood transfusion.
