Abstract (EN)

Introduction: Hypervolemia is one of the most important complications of acute kidney injury (AKI). Early diagnosis and treatment is vitally important because of morbidity and mortality associated with AKI. In this context, with bioelectric impedance device can be used to identify the body fluid configuration recently. The aim of this study is to detect body fluid status with bioelectrical impedance analysis in patients that renal damage hasn?t occurred previously and to evaluate correlation with the vena cava inferior(VCI) diameter and the other volume indicators such as N-terminal brain natriüezis peptit(NT-proBNP). Material ? Method: Between 2011-2013 years, this study included 100 patients with Acute Kidney Injury who were hospitalized and treated in Dicle University Medical Faculty Nephrology Clinic and Intensive Care Unit of general internal medicine clinic. The study is based on relative hydration status (RHS) is one of measurement parameters of Bioelectric impedance device. Relative hydration status is overhydration (OH)/ extracellular water (ECW) with the rate as determined by the device. Patients were divided into two groups in group 1: RHS below 15 % , group 2: RHS more than 15 %. Patients biochemical, hematological parameters were taken. VCI diameter determined with ECO and NT-proBNP levels were measured in all patients. Findings: In our study, %47 patients were male and %53 are women and in those patients %57 prerenal, %35 renal , %8 postrenal AKI. The groups have occurred group 1: 51 ,group 2: 49 patients. Between the two groups in the parameters of the urea, creatinine, Na, K, P, ICW, SBP, DBP, EF, and LAD parameters was no significant difference as statistically. In group 2 according to the group 1 the age , albumin and urine volume were lower but CVP, OH, RHS, TBW, ECW, VCI diameter and LogNTproBNP levels were higher in this group. There were significant positive correlations between the RHS and SBP, TBW, ECW, LogNT-proBNP and VCI, significant negative correlation was found between age and albumin. In this study we were found the strongest positive correlation with RHS and TBW (p = 0.001), the strongest negative correlation (p = 0.021) was found with albumin . Conclusion: 49 % of patients with acute renal ınjury was found to be RHS?15%. RHS showed a positive correlation with measurements of NT-proBNP and VCI. RHS positively correlated with total body water and negatively correlated with serum albumin value. RHS which is one of the measurement parameters of bioelectrical impedance analysis, is seen as an effective method of showing the configuration of the body fluids .
