Abstract (EN)

THE EVALUATION OF THE CONCENTRATIONS OF sIL- 1RII, sTNF-αRIAND sIL- 6R IN SYNOVIAL FLUID IN TEMPOROMANDIBULAR JOINTINTERNAL DERANGEMENTABSTRACTThe etiology of temporomandibular joint internal derangements, that isfrequently seen in population, is occupied with disk position, intraarticularvacuum effect, alterations in synovial fluid and degenarations of articularsurfaces. Recently, the effects of alterations in synovial fluid andproinflammatory cytokines are more popular. Especially, IL- 6, TNF-α, IL- 1βand their receptors are noticeable. This study is planned for evaluating theeffects of sIL- 6R, sTNF-αRI and sIL- 1RII concentrations in synovial fluid of thepatients with temporomandibular joint internal derangements on the successrate of arthrocentesis. The soluble receptors like sTNF-αRI and sIL- 1RIIpromote the effects of their cytokine molecules, but sIL- 6R adversely affects onits own cytokine molecule. In this study, 23 patient, who were complained aboutlimited mouth opening and pain during jaw function, underwent arthrocentesisand the synovial fluid samples which were aspirated before operation wasexamined. Maximum mouth openings of each patient were measured beforeoperation, early after operation, at the first week and on the first month ofoperation, and visual analog scale(VAS) was used for measuring pain levelsduring jaw functions. In biochemical analysis, specific Eliza kits for eachreceptor was used. A maximum mouth opening of more than 38mm anddecreasing in VAS scores were arbitrarily taken as indicating the success oftreatment. In conclusion, we observed that the concentrations of sTNF-αRI andsIL- 1RII in successful group were not statistically different from which inunsuccessful group, but the concentration of sIL- 6R in successful group wasstatistically higher than unsuccessful group.Key Words: Internal derangement, proinflammatory cytokines, arthrocentesisvii
