Evaluation of the relationship between platelet lymphocyte ratio and i̇nflammation in patients with anca associated vasculitis

ANCA associated vasculitis are complex-immune mediated disorders in which tissue injury results from an initiating inflammatory event and a higly specific immune response to this event. Methods like the Birmingham Vasculitis Activity Score (BVAS) have been developed to assess disease activity. In our study, we aimed to determine the PLR, NLR ratios in patients diagnosed with AAV and to evaluate retrospectively the relationship between these values and disease activity. In this study, 123 patients followed up with the diagnosis of ANCA-related vasculitis in our center between 2004-2022 and 50 patients followed in the hypertension outpatient clinic were included as the control group. Age, gender, date of diagnosis, ANCA serology, admission creatinine, leukocyte, neutrophil, lymphocyte, hemoglobin and platelet, albumin, sedimentation, CRP levels, clinical findings that is associated with the disease until the time of presentation; kidney involvement, lung involvement, general symptoms, skin involvement, mucosal membrane involvement, ear, nose and throat involvement, neurological involvement, cardiac involvement, gastrointestinal system involvement were recorded. Based on these data, BVAS, PLR ratios and NLR ratios were calculated at the time of diagnosis. It was observed that there was a significant correlation in the same direction between BVAS measurement and PLR level (r=0.27; p=0.008). It was determined that there was a significant correlation between the level of BVAS and the level of NLR (r=0.34; p<0.001). It was determined that PLR and NLR levels were also in a significant relationship with CRP level (r=0.22; p=0.015 and r=0.29; p=0.001, respectively). There was no statistically significant difference in PLR values between GPA and MPA subdiagnosis groups (p>0.05). In our study, PLR and NLR ratios were found to be correlated with CRP and BVAS in patients with AAV, and it was predicted that it could be used as a marker to show disease activity or the severity of inflammation.
