Comparative evaluation of classic colporraphy anterior and anterior repair with mesh in surgical repair of anterior vaginal wall

Abstract (EN)

The aim of successful surgical treatment of POP is; to relieve from the symptoms, to rectıfy the anatomy, to obtain subjective cure, to increase sexual and visceral functions. We had studied the treatment of POP anterior vaginal compartment defects. And in this paper, we had compared the short term results on complications and benefits of life quality that is obtained in the treatment of using classical Colporraphy anterior as well as a new operation method, `Anterior repair with mesh? .This study was conducted between June 2009 and December 2010 over 37 patients admitted to our clinic with diagnosis of symptomatic anterior vaginal compartment defects (stage 2 or higher). On 20 patients Colporraphy anterior and/or additional operations were applied and called `Control group?, on the remaining 17 patients Anterior Repair with Mesh and/or additional operations was applied. Questionnaire forms (PISQ-12, PFIQ-7, PFDI-20, PSI-QOL forms) evaluating demographic characters, history, quality of life and symptoms were filled by the patients. During physical examination pelvic organ prolapsus was evaluated according to POP-Q scoring system. Operations type and duration, intraoperative and postoperative complications, hospitalization period and patient satisfaction were recorded. Post-operative questionnaires were re-filled in. Patients were re-evaluated at 1st and 6th weeks.In Mesh group, satisfaction rates of the patients were significantly high (p=0,001). In both groups of patients, anatomic relief and contribution to quality of life were similar. Although, statistically not significant, the anatomic cure rates were found to be 82% in Mesh group and 60% in Control group. Subjective cure rates were determined as 100% for Mesh group? and 70% for Control group. In Mesh group, operation time was confirmed to be significantly longer than Control group (p=0,001)In the treatment of anterior compartment defects, though Colporraphy anterior and `Anterior repair with mesh treatment?s short term success rates were similar, anterior repair with Mesh treatment seems to be advantageous with higher patient satisfaction as well as higher subjective cure rates.
