The effect of changes in sagittal spinopelvic parameters on functional results in surgical treated patients with scheuermann kyphosis

Abstract (EN)
Scheuermann's kyphosis is the most common cause of thoracic kyphosis in adolescence. It starts in early adolescence and is seen as a progressive deformity with growth attack. In this study, we retrospectively evaluated 15 cases with scheuermannn kyphosis who were surgically treated in Celal Bayar University School of Medicine Department of Orthopaedics and Traumatology between 2010 and 2017. In our study, we analyzed the distribution of patients according to age, gender and Risser stages. Kyphosis angles, pelvic parameters and correction rate were measured. Complication rates and quality of life questionnaire scores were calculated. There was no significant correlation between them. In order to understand the relationship between sagittal vertical axis and the difference between the angular measurement parameters that affect sagittal balance positively and negatively, no correlation was found between the measured values and SVA. However, there was a significant decrease in SVA values in all cases. DJK did not develop in any of the cases during our measurements. When the distal instrumented vertebra levels were evaluated, ILV and SSV levels were observed in all cases. Since junctional kyphosis did not develop; No correlation between the development of DJK and SEV, and between DJK and SSV-ILV were observed. We found only significant changes in the values of thoracic kyphosis and lumbar lordosis between preop and postop measurements. No significant changes were observed in pelvic parameters. When SRS-22r and SF-36 questionnaires were applied to the patients, it was observed that the patients were very satisfied with the treatment in general. No correlation was found between functional scores, angular correction values and radiological parameters such as DJK. There was no significant difference between the study patients who had criteria such as quality of life questionnaire, pain and social functioning psychological evaluation. No significant difference was found between the rest of the group in parameters such as spine function, functional score and body image. No postoperative loss of correction was observed. Postop quality of life and correlation rates were found to be consistent with studies in which all levels were instrumented. Our study supports the need for surgical treatment in patients with surgical indications of Scheuermann's kyphosis and the adequacy of correction with posterior instrumentation.
