Adaptation of childre's depression rating scale-revised in Turkish and it's psychometric properties in Turkish adolescents

Objective: Because it has several difficulties to determine how to diagnose childhood depression, it has been needed reliable different evaluation enstruments to decide the existence and severity of this disorder. Children Depression Rating Scale-Revised (CDRS-R) is an estrument that is evaluated by a clinican and it needs bringing up the information from child/adolescent, parents and clinical assesment together. In clinical population, CDRS-R provides general information about depressive symptomatology and severity of depression also. It provides information not only during diagnosis but also during the course of disease and it?s treatment response.The aim of this study is to translate and adaptation of CDRS-R in Turkish and evaluate it?s psychometric properties in adolescence in Turkish population.Because of this purpose CDRS-R has been translated and retranslated. Turkish form has been established at the end of the translation process. It has been determined internal consistancy, interrater reliability, construct validity, discriminant validity and criterion validity of CDRS-R Turkish form. The information were derived from the visits in which CDRS-R Turkish form used, compared with the information from Schedule for Affective Disorders and Schizophrenia for School Aged Children, Present and Lifetime Version (K-SADS P/L) evaluation, Beck Depression Scale, Clinical Global Impression-Severity Form (CGI-S), Children?s Global Assesment Scale (CGAS).Method: This study was designed with 76 adolescents. 38 adolescents within range 12-18 of age; that were diagnosed as Major Depressive Disorder (MDD) and 38 adolescents within range 12-18 of age; who had no axis I disorder indeed. It had been performed both K-SADS P/L and CDRS-R clinical interviews with all subjects and applied socio-demograpich data form, considering of their age Beck Depression Scale (14-18) or Children Depression Inventory (8-14), both CGAS and CGI-S to all. All the evaluations were performed by the clinicans. After the parents and the child/adolescent had been informed, the CDRS-R interviews were recorded to achieve the second clinican to grade CDRS-R for the evaluation of interrater reliability.Findings: With all data we had provided from our study, we evaluated CDRS-R?s psychometric properties for Turkish adaptation in Turkish adolescents. From the data we had provided from the population who had diagnosed as MDD, we found the value of Cronbach?s ? (0.876) was highly reliable. To determine interrater reliability we calculated Intraclass corealation coefficents(ICC) for MDD diagnosed group. We found ICC between the range of 0.92 to 0.99 for each item that means interrater reliability was found statistically significant and highly correlated. Each of CDRS-R items were significantly correlated with eachother in the group of MDD diagnosed patients.For validity evaluation, it had been performed factor analysis for constract validity, we had managed to identify 16 factors that?s eigenvalues were bigger than one. Varimax rotation was performed and four factors were evaluated. It has been observed that CDRS-R?s items common variables were within the range of 0.45 to 0.87.To support the construct validity of the CDRS-R, CGI-S and CGAS points that were determined by the clinican during the interview were compared with the CDRS-R Raw score; and they were found highly correlated.To determine paralel form validity we performed statistical analysis. We found Beck Depression Scale?s sensitivity was 90.9%, and spesifity 87.8% and positive predicitive value was 88%, negative predictive value was 90%. We found 100% both sensitivity, specifity, positive and negative predictive values for CDRS-R that were significantly high.Conclusion: In our study we evaluated CDRS-R?s psychometric properties in Turkish adolescents and in this context we performed it?s Turkish adaptation and identified the preliminary findings of validity and reliability. We found that CDRS-R Turkish version has reliability and validity for using the diagnosis and rating of severity of MDD in Turkish children and adolescents.Key Words: Major Depressive Disorder, Adolescent, Children?s Depression Rating Scale-Revised
