The relationship between vitamin D level and clinical andlaboratory parameters in chronic kidney disease

Abstract (EN)

CKD is defined by the presence of GFR<60mL/min and/or GFR independent markers of kidney damage lasting more than three months. Vitamin D deficiency is an important problem both in society and in CKD patients. Increased FGF-23 secretion in CKD, malnutrition, urinary excretion of vitamin D-binding protein, suppression of 1-α hydroxylase activity are possible factors affecting this.. Vitamin D deficiency increases as the stage of CKD progresses. There are many studies showing the effects of Vitamin D on proteinuria, inflammation, insulin resistance and the cardiovascular system. 160 patients with CKD who came to our clinic and were tested for vitamin D were included in our study (82 men, 78 women). Data were scanned retrospectively. To be used in the study through the hospital database, patients; Information on age, gender, comorbidities (diabetes, hypertension, etc.), GFR, vitamin D, calcium, phosphorus, PTH, Hba1c, CRP, albumin, proteinuria amount and whether dialysis was performed or not was obtained. Vitamin D level was taken as the first value checked in the hospital and could not be included in the study because follow-ups were not regular after replacement. Other laboratory parameters are the values on the dates when the vitamin D level was taken. As a result of our study, no significant relationship was found between CKD stages according to GFR and vitamin D level. (p = 0.235). Again, no significant difference was found between the primary disease and vitamin D levels (p = 0.447). A significant relationship was found between the amount of proteinuria and vitamin D level of the patients (p = 0.001). A significant relationship was found between CRP levels and vitamin D, and between vitamin D and PTH, calcium phosphorus and albumin levels (p = 0.024, p = 0.020, p = 0.025, p = 0.001, respectively). No significant relationship was found between vitamin D and other variables. As a result, our study emphasizes the importance of the relationship between CKD and vitamin D, but more comprehensive studies are needed on this subject.
