Comparison of scapular position, shoulder joint position sense and upper extremity strength in individuals with chronic neck pain and healthy participants

Abstract (EN)
In our study, we planned to evaluate the difference between the two groups by evaluating the scapular position, shoulder joint position sense and upper extremity strength in individuals with chronic neck pain as well as among healthy participants. Individuals with cronic neck pain and healthy participants who met our criteria in a Private Physical Therapy and Rehabilitation Center in Ankara and who were admitted because of the number of chronic neck pain problems were included in the study. In this case-control study, 23 patients and 23 controls were included. Healthy participants with similar physical characteristics (aged 20-50 years) and who also suffered from chronic neck pain for at least 6 months, and who have a pain intensity of 3 according to the VAS, were included in the study. The lateral scapular slide test (LSST) was used for the clinical evaluation of the scapular position in the frontal plane using a tape measure. Shoulder joint position sense has been measured using an inclinometer. For evaluation of the upper extremity force, the grip strength of the individuals was measured using a dynamometer. Data were analyzed by appropriate statistical methods. In the patient group, the mean difference between LSST 0° on the right and left sides was higher than that of the control group (p = 0.009). Similar results were obtained for 45° and 90° measurements; during which the difference between the right and the left sides in the patient group was higher than the control group (p <0.05). Although the upper extremity force of the dominant and non-dominant sides was proven to be higher in control group, no significant difference was observed with the patient group (p> 0.05). In the evaluation of shoulder joint position sense, the values obtained for the levels of deviation in the abduction movement of both shoulders was higher in the patient group compared to the control group (p <0.001). Similarly, the values obtained for the levels of deviation in both sides during flexion movement was higher in patient group (p <0.001). As a result of our study, it was observed and concluded that the position of the scapula was changed with patients suffering from neck pain compared to healthy subjects, additionally, differences in the shoulder joint position sense was noticed in patients. However, there was no change in the strength of the upper extremity muscles. Accordingly, we think that it would be beneficial to include scapular smoothness and to develop shoulder joint position sensation in atients with chronic neck pain in physiotherapy process.
