Abstract (EN)

Aim: The aim of our study is to evaluate the cardiac changes in childhood obesity, assesserum adiponectin and resistin levels, relationship between cardiac changes and toserum adiponectin, resistin levels and insulin resistance in childhood obesity.Materials and methods: Seventy one obese children (44 boys, 27 girls, mean age12.1±2.6 ages, age and gender specific BMI>97 percentage), 24 overweight children (6boys, 18 girls, mean age 12. 0±1. 9 ages, age and gender specific BMI: 85?97percentage) and age and sex matched 40 control children (21 boys, 19 girls, mean age11.1±2.3 ages, age and gender specific BMI<85 percentage) were studied. Twodimensional,M mode, pulsed Doppler and tissue Doppler imaging echocardiographywere used. Serum adiponectin and resistin levels were measured by ELISA, fastinginsulin levels measured by Immunuassay. Results: Serum adiponectin and resistin levels were significantly lower in obese andoverweight group than control group (respectively p<0,001; p<0,05). There was norelationship between serum adiponectin, resistin levels with fasting insulin, HOMA-IR.There was not any significant difference in adiponectin and resistin levels betweenobese and overweight children with insulin resistance and obese and overweightchildren without insulin resistance. In echocardiographic evaluation; obese andoverweight group had significantly higher LV mass and LV wall thickness than controlgroup. There were diastolic dysfunction signs in obese and overweight group, when thepatient evaluate with Pulsed Doppler and tissue Doppler Echocardiography. In obesegroup there was signicantly correlation between serum resistin levels, fasting insulin,HOMA-IR with LV mass. Adiponectin levels did not show any correlation with cardiacparameters, fasting insulin and HOMA-IR.Conclusion: This study demonstrated that, obese and overweight children havesignificant changes for left ventricular wall dimensions, LV mass and early diastolicfilling features when compared with nonobese children. Obese and overweight childrenhave significantly lower serum adiponectin and resistin levels than controls. There is arelationship between serum resistin, fasting insulin and HOMA-IR with LV masssuggesting that these factors might influlence the cardiac remodelling in obesity withincreasing LV mass. We did not find any relationship between serum adiponectin levelsand cardiac parameters, and there was no correlation between resistin and adiponectinwith insulin resistance.
