Retrospective investigation of the role of RENAL, PADUA and SPARE nephrometry scores in predicting perioperative and postoperative surgical outcomes in renal masses undergoing partial nephrectomy

Abstract (EN)

Retrospective investigation of the role of RENAL, PADUA and SPARE nephrometry scores in predicting perioperative and postoperative surgical outcomes in renal masses undergoing partial nephrectomy. Partial nephrectomy is the leading treatment method in patients who are suitable for the surgical treatment of kidney masses. Nephrometry scores have been developed to be used in appropriate patient selection for successful perioperative and postoperative outcomes. In our study, the success of RENAL, PADUA and SPARE scores was investigated. The data of 419 patients who underwent partial nephrectomy between 2007-2020 were retrospectively analyzed. The mean age was 55.89 (± 12.5) years, the female/male ratio was 38.4%-61.6%, and the mean follow-up period was 10 ± 7 (1-51) months. The RENAL score was significant for the duration of the operation, the amount of bleeding, and intraoperative complications in univariate analysis (p:0.009, p:0.001, p:0.003); the PADUA score was significant for the amount of bleeding and postoperative complication in univariate analysis and was significant for intraoperative complication, surgical margin positivity, trifecta success and pentafecta success in multivariate analysis (p:0.005, p:0.001, p:0.001, p:0.014, p:0.002, p:0.009); the SPARE score was significant for the amount of bleeding, trifecta success and pentafecta success in univariate analysis and was significant for intraoperative complications in multivariate analysis (p:0.002, p:0.001, p:0.027, p:0.049). Intraoperative complications was significant for the need for ischemia, tumor location, tumor exophytic ratio, collecting system and renal sinus involvement in univariate analysis (p:0.036, p:<0.001, p:<0.001, p:<0.001, p:<0.001). Surgical margin positivity was significant for tumor size in univariate analysis and was significant for Fuhrman grade in multivariate analysis (p:0.013, p:0.007). It was observed that only PADUA score (p:0.002) with trifecta success, and PADUA score and tumor size with pentafecta success in multivariate analysis (p:0.009, p:0.025). The nephrometry scores evaluated in our study are easily applicable methods that can be used to predict perioperative complications and to select suitable patients for partial nephrectomy.
