Prognostic impact of intraductal carcinoma presence in specimen pathology of patients who undergone radical retropubic prostatectomy due to prostate cancer

Abstract (EN)
Objective: We investigated the prognostic significance of intraductal carcinoma of prostate (IDC-P) and the impact of IDC-P on biochemical failure and adjuvant therapy planning in radical prostatectomy (RP) specimen pathology which shows extraprostatic extension (pT3a) with negative surgical margin and organ confined (pT2) with positive surgical margin. Method: We retrospectively evaluated 67 patients who were perfomed RP between 2000- 2014 and whose pathology results are pT3a with negative surgical margins (group 1, n=35) and pT2 with positive surgical margin (group 2, n=32). RP specimens were re-evaluated and the specimens which has IDC-P component were seperated. In both groups, presence of IDC was compared with other prognostic parameters and biochemical recurrence. Results: In group 1, IDC was detected in 5 cases and biochemical recurrence was detected in 3 cases. Patients with IDC-P showed a more biochemical failure than those without IDC-P (p=0,002). In univariate analysis, IDC-P was significantly associated with progression free survival (p<0,001). In group 2, IDC was detected in 4 cases and biochemical recurrence was detected in 10 cases. Patients with IDC-P showed a higher tumor volume than those without IDC-P (p=0,02). IDC-P was significantly associated with progression free survival (p=0,033). Conclusions: In both groups IDC is a prognostic factor for progression free survival. In these cases, presence of IDC might be helpful for postoperative adjuvant therapy management.
