Detection of pancreatic beta cell reserve and its relationship with metabolic markers in patients with active psychotic and mood disorders

Abstract (EN)
Metabolic syndrome with a multifactorial pathogenesis is also known to be associated with atypical antipsychotic drugs, which are widely used in psychiatry. It was observed that not only increased mortality, but also psychotic and depressive symptoms, adherence to treatment and functionality decreased in patients with metabolic disorders. However, atypical antipsychotics are associated with varying degrees of weight gain, metabolic abnormalities such as diabetes and hyperlipidemia. In this study, we aimed to investigate the determination of pancreatic beta cell reserve and its relationship with metabolic markers in patients with psychotic and mood disorders, who are undergoing active treatment. Sixty patients who applied to the psychiatry outpatient clinic and whose active treatment continues were included in the study. The healthy control group, consisting of 60 people, was composed of people who applied to the endocrinology outpatient clinic for routine check-up and had no medical illness. Chi-Square analysis (Pearson Chi-square), Kolmogorov-Smirnov test, Student t-test, Mann Whitney-U test, Spearman Correlation test were used in data analysis. SPSS 22 packaged software was used in the analysis of the data. The significance value was taken as p≤ 0.05. In the case group, weight, BMI, waist circumference, neck circumference, hip circumference, waist / neck ratio and waist / hip ratio were found to be significantly higher than the control group (p<0,05). HOMA-IR value of the case group was found to be significantly higher than the control group (p=0,041). However, there was no significant difference between the case and control groups in terms of FPG, insulin, C-peptide and HbA1c values (p> 0.05). TG, VLDL, T. cholesterol and LDL values of the case group were found to be significantly higher than the control group. In the correlation analysis, a significant positive relationship was found between weight and BMI, waist circumference, waist / neck, waist / hip, insulin and c-peptide. As a result of the studty, significant increase in most metabolic markers was found in patients with psychotic and mood disorders, and with continuing active treatment. We are of opinion that the cases which have high risk factor should close follow-up in terms of obesity, insulin resistance, hyperlipidemia, prediabetes and diabetes will be beneficial.
