Abstract (EN)
Aim: Several studies have emphasized that cicatricial alopecia (CA) is a significant health problem, especially in the middle-aged population. We aimed to evaluate the data of patients diagnosed with CA who presented to the Dermatology and Venereal Diseases outpatient clinic of a tertiary referral university hospital with complaints of hair loss. Materials and Methods: In this study, the data of patients diagnosed with CA, who presented to the Dermatology and Venereal Diseases outpatient clinic of Istanbul University-Cerrahpaşa, Cerrahpaşa Medical Faculty, between 15.12.2020 and 15.03.2024 with complaints of hair loss, were retrospectively analyzed. The patients' demographic data, Fitzpatrick skin types, comorbidities, dermatological examination findings, disease durations, disease severities, involvement percentages, and involvement areas were noted. Furthermore, CA subtypes and the medical treatment modalities applied to the patients were evaluated. The patients were divided into remission and active disease groups, and their data were compared. Results: Our study included the data of 308 patients with a mean age of 39.1 ± 12.6 years. Seventy-three patients (23.7%) were diagnosed with folliculitis decalvans, 62 (20.1%) with lichen planopilaris (LPP), and 54 (17.5%) with perifolliculitis capitis abscedens et suffodiens. The mean age of disease onset was calculated as 35.3 ± 10.8 years. The median disease duration was 3 years (2 - 5 years). When evaluating the severity of the disease, 119 patients (38.6%) had mild alopecia, 162 patients (52.6%) had moderate alopecia, and 27 patients (8.8%) had severe alopecia. Regarding the areas of involvement, 65 patients (21.1%) had involvement of the frontal area, 80 patients (26.0%) the parietal area, 130 patients (42.2%) the occipital area, 113 patients (36.7%) the vertex area, and 47 patients (15.2%) had involvement in more than one region, including the temporal area. The mean age of patients in the active group (35.3 ± 11.2 years) was statistically significantly lower than that of the remission group (44.5 ± 12.5 years) (p=0.001). The median value of the involvement percentage was calculated as 20% (10 - 20) in the active group and 10% (5 - 20) in the remission group(p=0.001). Regarding the severity of the disease, 26.3% of patients in the active group had mild, 59.2% moderate, and 14.5% severe alopecia, whereas in the remission group, 55.8% of patients had mild, 43.4% moderate, and 0.8% severe alopecia (p=0.001). Conclusion: In our study, the most common CA subtypes were folliculitis decalvans, LPP, and perifolliculitis capitis abscedens et suffodiens. CA was most frequently diagnosed in middle-aged patients, with a mean disease duration of three years. The occipital region was identified as the most commonly affected area. When comparing patients in the active and remission groups, those with active disease were significantly younger, had an earlier onset of the disease, and had a significantly higher percentage of involvement and disease severity than those in remission.
