Comparative study of the effects of invisalign and monoblock applications in the treatment of class II div 1 cases

Abstract (EN)

The aim of our study is to examine and compare cephalometrically the skeletal, dental and soft tissue changes of functional treatments in Angle Class II individuals in the growth and development period. In this prospective study, the group treated with Invisalign MA clear aligners and the patient group consisting of Class II monoblock patients obtained from archival material were compared. The first group consisted of 10 individuals (mean age 10.50±1.13 years) treated with clear aligners, and the second group consisted of 10 individuals (mean age 10.75±0.89 years) treated with monoblock. In the selection of individuals, attention was paid to the fact that all individuals had a functional Class II malocclusion, Angle Class II molar and/or canine relationship, the development pattern was not high angle, they were in the growth period and there was no history of orthodontic treatment. The first group was treated with clear aligners for an average of 12 months, while the second group was treated with a monoblock for an average of 12 months. To evaluate the treatment effects, T0 and T1 materials obtained before appliance application (T0) and after appliance removal (T1) were used in Groups 1 and 2. Wilcoxon Sequential Signs Analysis Method was used to compare the changes between T0-T1 periods in the evaluation of the measurements made on lateral cephalometric x-rays, and the Mann Whitney U test was used to compare the difference measurements according to the groups. The statistical significance level was taken as p<0.05 in the calculations. A significant decrease in ANB was noted with Invisalign MA clear aligners and Class II monoblock appliances, with a non-significant decrease in SNA and a significant increase in SNB value. While the SN-GoGn value increased significantly with the Class II monoblock appliance, the mandible (B-VR value) increased significantly with both appliances. In both groups, a significant increase in effective mandibular length (Co-Gn) and a significant decrease in overjet and overbite values were observed according to the treatment results. While a significant retrusion and retroclination occurred in the upper incisors in both groups; in the lower incisors, a significant protrusion and proclination occurred in the monoblock group, and a significant retrusion and retroclination occurred in the group treated with Invisalign MA clear aligners. As a result, positive dental, skeletal and soft tissue changes can be achieved with Invisalign MA featured clear aligners and Class II monoblock appliance in the early treatment of Class II malocclusion with appropriate case selection; with Invisalign MA clear aligners, dental corrections can be made simultaneously with skeletal effects, thereby effectively shortening the total treatment time.
