Effects on intra-operative music listening on intraoperative vital signs, postoperative pain and anxiety in patients undergoing spinal anaesthesia

Abstract (EN)
This study aims to investigate the effect of music played during surgery on intraoperative vital sign and postoperative pain and anxiety at spinal anesthesia. The study was carried out in the operating rooms of Training and Research Hospital. The patients in Orthopedics and Traumatology Clinic, who underwent elective orthopedic surgery and had spinal anesthesia and knee prosthesis. This randomized controlled study was conducted with case/music group by 22 patients, sedated control group by 25 and without sedation group by 22 patients. After spinal anesthesia and until the end of surgery, music was played through the earplugs which closed all the ears. After the spinal anesthesia, doctor applied sedation in sedated patient group. The patient group without sedation was followed without any application from spinal anesthesia was applied to the end of the operation. Data analysis was conducted Patient monitoring during the operation and collected by the researcher in face-to-face interview technique. Data analysis was conducted using the SPSS software package (version 20.00) by number, percentage, mean, one-way anova, kruskal wallis, wilcoxon signed rank test and MANOVA tests. There was no statistically significant difference between the case musical group, sedated and non-sedated control groups in terms of age, gender, presence of chronic illness and previous surgery under spinal anesthesia (p>0.05) and the groups were homogeneous. It was found that the majority of TDP patients have a mean age of 66.8, 84.1 womens patients with 32.6 VCP, 68.1% with chronic disease, 78.3% with ASA II and 49.3% of patients with left TDP. The majority of patients in the case group (%68.2) preferred to mystic music and others (%31.8) folk music during surgery. Classical music and ocean sound were not preferred. During the operation, the number of pulses in the sedated control group was lower at the 90th and 120th minutes and respiratory numbers were higher at the 30th and this was statistically significant (p <0.05), Although there was no statistically significant difference (p>0.05) in other life forms follow-up, the pulse, systolic and diastolic blood pressures of the patients in the case group were found to be more stable. No complication was observed in the case group during perioperative period. However; intubation indications due to suppression of bradycardia, hypotension and respiratory system in the sedation group and hypertension in two patients and tachycardia in one patient were observed in the control group without sedation. The pain score in the musical group ( :5.2) was found to be lower than the pain score in the control group without sedation. There was no difference between preoperative and postoperative state and continuity anxiety scores of the patients. There was no change in the state anxiety scores of the patients in the music group. It was observed that the anxiety scores increased in the other groups. It was determined that 63.6% of the patients in the control group without sedation were uncomfortable to hear the sounds during the operation. The patients reported that in the music group were comforted by listening to music and 52.0% of the patients in the sedated control group were not disturbed by the sounds. As a result; music therapy is a cost effective method that can be used in reducing the anxiety and pain of patients and positively affecting the life signs and in surgical procedures performed with regional anesthesia which does not require side effects and does not require patient cooperation.
