2.ABSTRACTCOMPARISON OF VENLAFAXIN WITH PLASEBO FORMEDICAL MANANGEMENT OF STRESS URINARY INCONTINANCEOBJECTIVE: Venlafaxin is a dual inhibits reuptake of both serotonin andnoradrenalin and stimulates pudendal motor neuron (Alfa)-1 adrenergic and 5-hydroxytryptamine-2 receptors so stimulates urethral muscles (rhabdosphincter)contractility. We examine the effectiveness and reliability of venlafaxin for medicaltreatment of stress urinary incontinence patients.MATERIAL AND METHODS: Forty patients aged 24-52 years included into thedouble blinded placebo controlled study. Criteria for inclusion were, marked SUIsymptom, incontinence episode frequency for 7 or above per weekly, without urgeincontinence symptoms, normal diurnal and nocturnal miction frequency, bladdercapacity 400 cc or more, Boney and Q type positivity, positive cough and positive stresspad tests. Daily urinary forms refilled up by the patients two before treatment and threeafter treatment. Patients randomly divided into two groups by applying them life qualityforms. Seventy-five mg (1x1) venlafaxin (G1, n=20 patient) and placebo (G2, n=20patient) administered to the patients orally for 12 weeks.RESULTS: Socio-demographic characteristic and SUI symptoms were similar amongall patients. Basal week IEF value was 17. Marked decrease at IEF scores (83% vs.25%, p <0, 01) and marked increase at I-QOL scores (50% vs. 9%, p <0, 01) obtainedstatistically when venlafaxin group compared with placebo. Significant increaseobtained when VI compared between venlafaxin and placebo group (24 minutes vs. 5minute, p <0, 01). PGI-I results were markedly lower in venlafaxin group than placebo3group (72% vs. 10%, p <0, 01). Side effects compared between groups and nausea wasmarkedly seen in venlafaxin group (40 vs15 %, p <0, 01).CONCLUSION: Venlafaxin is a pharmacologic agent that has a treatment potential atSUI patients. It may be an alternative method for medical management of SUI.KEY WORDS: Stress urinary incontinence, venlafaxin, serotonin noradrenalinreuptake inhibitor, I-QOL, PGI-I, PGI-S, IEF, VI.4
