Effects of nursing care given to adolescents with type 1 diabetes in accordance with the health promotion model on the outcomesof patient care

Abstract (EN)

Objective: This study aims to determine the effects of home-based nursing interventions performed in accordance with the health promotion model on the outcomes of patient care by taking into account obstacles and requirements of adolescents with type-1 diabetes. Method: The study will consist of two stages. The first stage is a qualitative study to determine the home care needs and barriers of adolescent with type 1 diabetes. Semi-structured interviews were conducted with 18 adolescents with type 1 diabetes. Data were analyzed with content analysis by researchers. The second stage was carried out with an experimental design by using Health Promotion Model. The study was completed with 36 adolescent with type 1 diabetes in experimental group and 35 in control group. The data were collected using by data form adolescent with type 1 diabetes, Diabetes Behavior Rating Scale, General Self-Efficacy Scale and metabolic control data form. . In the analysis of the data, chi square, the t test, variance regression and structural equation model analysis were used. Results: The barriers perceived by adolescents with Type 1 diabetes in maintaining the behaviours needed to manage the disease effectively consisted of three themes including 'individual's attitudes and beliefs', 'environmental effects', and 'provision of healthcare services' and the sub-themes related to these. After home nursing interventions, there was a significant difference between experimental group and control group (p<.05). HbA1c, diabetes behavior responsibility, self-sufficiency levels levels were decreased in experimental group (p<.05). ); there was no difference with HbA1c, fasting blood glucose, body mass index, (p>.05). There was a significant difference appliying hospital for a complication and cost averages between groups (p<.05). Conclusion: For development of type 1 diabetes self-management skills, improved the glisemic control and nursing interventions are presented to the diabetics according to their individual needs and periodic control is recommended.
