Abstract (EN)

Patellar fracture, patellar and quadriceps femoris tendon ruptures causing distruption in continuity of extensor mechanism as a complication in total knee arthroplasty are seen rarely but when seen success of treatment is low and effects patients quality of life adversely. Purpose of this thesis is to study the factors responsible for distruption in continuity of extensor mechanism after total knee arthroplasty.405 knees of 244 patient having total knee arthroplasty is studied. Factors responsible for distruption in continuity of extensor mechanism after total knee arthroplasty evaluated seperately as factors depending on patients and knees.Increasing patient age as a patient depended factor is related to distruption in continuity of extensor mechanism and it is statisticaly significant (p=0,046). The functional knee score measured in the last control of patients, a knee depended factor, is seemed to be protective for distruption in continuity of extensor mechanism significantly (p=0,009). Also effect of fluoroquinolone use is highly related to patellar and quadriceps tendon ruptures after total knee arthroplasty (p<0,001). Risk of patellar and quadriceps tendon ruptures in patients using fluoroquinolones are increased by 6,95 folds.The effect of age and functional knee scores after total knee arthroplasty on extensor mechanism problems have been studied before but no relation have been reported. Although tendinopathies and spontaneous tendon ruptures related to fluoroquinolone use are reported in literature, there is no reports on fluoroquinolone use and patellar and quadriceps tendon ruptures after total knee arthroplasty. Distruption in continuity of extensor mechanism after total knee arthroplasty is effected positively by increasing age and fluoroquinolone use and adversely by increasing functional knee scores. While planning for total knee arthroplasty, patient age and fluoroquinolone use must be considered. Continuing effective physical theraphy for longer duration after the operation will decrease posibility of distruption in continuity of extensor mechanism.Key words: knee, arthroplasty, total knee arthroplasty, treatment, patellar tendon, quadriceps femoris tendon, rupture, fracture, fluoroquinolone, complication.
