ABSTRACT Role of Presepsin in the Diagnosis of the Patients With Ventilator-Associated Pneumonia Introduction: Mechanical ventilation is vital for treatment of acute respiratory failure in the intensive care units(ICU). Ventilator associated pneumonia (VAP), one of the dreaded complications of mechanical ventilation, is an important risk factor for morbidity and mortality besides increasing costs and length of intensive care unit stays. This necessitates the early diagnosis and identification of high-risk patients with VAP requiring aggressive treatment. Aim: Aim of this study is to investigate whether the presepsin is a diagnostic biomarker for VAP and it has an effect on mortality ICU patients. Materials and Method: The present study was prospectively conducted in the Intensive Care Clinic of Department of Anesthesiology and Reanimation and Intensive Care Internal Medicine Department from November 2015 to July 2016 upon the approval of the Ethics Committee of Çukurova University School of Medicine. 60 patients who have been in the intensive care for more than 120 hours and who are over 18 years old and diagnosed with VAP have been included in our study. Exclusion criteria were HIV infection, active tuberculosis, immunosuppressive therapy, postsplenektomi and neutropenia in the study. On the first day of diagnosis of VAP, APACHE II score was calculated and recorded by using the worst values of 17 variables of APACHE II score. On the 1st, 3rd and 7th days, blood samples were collected from the patients included in the study in order to detect PRESEPSİN, WBC, CRP, PCT and Lactate values. Patients included in the study were monitored until 28th dayand the results were recorded as recovered, monitoring or exitus. Conclusion: It was concluded that Presepsin has no diagnostic impact in the patients diagnosed with VAP while APACHE II scores have prognostic impact for these patients and their increased results are associated with mortality.
