Abstract (EN)
The Effects of the Acute Physiologic Changes of the Patients Hospitalized In the Intensive Care Unit of the Psycology of the RelativesPurpose: The purpose of this study was to define the relationship between the change in the physiological states of the patients hospitalized in the Intensive Care Unit (ICU) and the symptoms of the anxiety and depression observed on the first degree family members of the patient.Materials and methods: Only one person who is 18 years older among the family members is involved in the study since the admission of the patient to the ICU. The reason of the admission to the ICU, ages of the patients and whether they have a chronic disease or not are all recorded. The heart rates and systolic arterial blood pressures of the patients are followed continuously. The consciousness levels are recorded through the Glaskow Coma Scale every day. Also, Harowitz ratio that is calculated with the proportion of the partial arterial oxygen pressure to the percentage of the inspired oxygen level, and the bicarbonate (HCO3) values are recorded. Blood leukocytes, urea, bilirubine, sodium and potasium levels are measured every day. All measurements about the patients are used in the Simplified Acute Physiology Score II (SAPS II) calculation. We defined the physiologic states of the patients and the severity of the diseases through the SAPS II score. Family members are informed about the states and the severity of the diseases of the patients by the ICU doctor before they have visited their patients. After the daily information, State Anxiety Scale for during seven day, Trait Anxiety Scale and Beck Depression Scale for the first and seventh days are applied to the relatives. Relatives how to be affected physicologically from the acute physiologic changes of the patients are determined.Results: While there was no correlation between the SAPS II Score and State Anxiety and seventh day Beck Depression Scale regarding as sex, only in women the corelation between the seventh day Trait Anxiety and SAPS II Scores is detected. According to the educational situation, there was a significantly great relationship between the SAPS II Scores of the patients and the State - Trait Anxiety and Beck Depression Scales in the primary school educated relatives and there was a low corelation in the university educated relatives. While the greatest corelation was found between the SAPS II Score of the patients and the State Anxiety Scale that was applied to the relatives in the parents, the corelation between the Beck Depression Scale and SAPS II Score is detected greatestly only in fathers. About the daily changes in the SAPS II Score and State Anxiety Scale were found with the significant corelation especially after the third day.Conclusion: Anxiety rises gradually especially in the women while the time passing. The anxiety and the depressional signs regarding as the relationship between the educational level are going to be reduced with the rising of the educational level of the relatives, after the obtaining information. At the same time while the comparing the young ages and the older ages about the relationship between the information and the signs of the depression and anxiety, there is a strong relationship in the older ages and the relationship is getting weak in younger ages.Key Words: Intensive Care Unit, Patient Relative, Anxiety and Depression
