Diagnostic value of neutrophil / lymphocite ratio in the difference of peripheral and power plant vertigo in patients administrating with emergency service

Abstract (EN)
Vertigo is one of the most common causes of emergency admission and can be easily directed to peripheral causes by history and physical examination taken by the emergency department physician. Differential diagnosis between central causes with high mortality and morbidity is necessary because of possible complications. Many studies have been conducted to assist physicians in differential diagnosis. In our study, the diagnostic value of neutrophil lymphocyte ratio (NLR) was differentiated between peripheral and central vertigo in patients presenting with dizziness in the emergency department. Patients admitted to the Emergency Medicine Outpatient Clinic of Karadeniz Technical University with the complaints of dizziness between January 1, 2017 and December 31, 2018 were included in the study. As a result of retrospective screening of patients with vertigo attack, 260 patients were included in the study according to the study criteria. With history and physical examination, peripheral and central vertigo separators were diagnosed by central imaging. Laboratory investigations of these patients were retrospectively compared and neutrophil lymphocyte ratios were recorded for differential diagnosis. Of the patients included in the study, 41.9% (n = 109) were male, 58.1% (n = 151) were female, and the mean age was 63.48 ± 17.61. Median NLR (median: 3.99) was significantly higher in patients with central vertigo than median NLO (median: 2.32) (p <0.001). According to the results of Magnetic Resonance Imaging (MRI), the area under the ROC curve was found to be 0.679 (95% CI: 0.600-0.7757) and this was reported to be statistically significant (p <0.001). If the NLR value is higher than 3,25, it is highly likely that there may be central vertigo and may be a guide in obtaining central imaging as additional imaging.
