The relationship between clinical, radiological and blood parameters of patients diagnosed with acute ischemic stroke in emergency room

Abstract (EN)

Aim: Cerebrovascular disease is the most common type of disease in neurological diseases. Ischemic stroke which is a type of cerebrovascular disease has a significant importance as it causes high mortality and morbidity. Ischemic stroke is the third leading cause of death in the world. Besides mortality it is also a cause of long term of disability in patients. The aim of the study was to assess the correlation between the clinical, radiologic and blood parameters of the patients who were diagnosed with acute ischaemic stroke. Method: This retrospective study included 383 patients presenting to the Emergency Department with ischeamic cerebrovascular disease between January 2015 and August 2016. Patients were analysed using available files and the Hospital Information Management System (Probel®); age, gender, patient history, physical and neurological examination Findings, history of chronical illnesses; Hgb, Hct, MCV, RDW, MPV, NLR, PLR values among the CBC parameters; cranial CT and diffusion – weighted MRI findings, echocardiography, carotid – vertebral system doppler usg results were obtained and analysed. Results: 201 (52,5%) of the 383 patients were male. The mean age of the patients was calculated as 67,45 ± 14,14. According to the complaints of the applicant and the physical examination findings of the study group, slurred speech was the most frequent finding, followed by weakness and motor sensory loss. Hypertension (HT) is the most common additional disease in patients' resumes. 377 of the patients had undergone cranial CT. Radiological pathology was detected in 254 of them. Cranial MRI was performed in 319 patients. While no pathology was detected in only 27 of the patients, the most affected area was the Anterior Cerebral Artery (ACA) supply area. The average Red Cell Distribution Width (RDW) was above the reference value range for both males and females. When hematological parameters were compared with cranial MRI; The RDW value was found to be statistically significantly higher in the group with abnormal MRI findings than in the group with normal findings. Conclusion: With the need for more extensive work to be done in the future; it can be considered that easily obtainable hemogram parameters such as MPV, RDW, NLR can be used more for emergency patients as predictive values in stroke patients.
