Abstract (EN)

ABSTRACT Cardiovascular diseases, including coronary artery disease and stroke, are the major causes of premature death in adults. Almost one in three deaths occurring before the age of sixty-five is due to cardiovascular disease. Acute coronary syndrome is defined as all clinical presentations caused by myocardial ischemia as a result of decreased coronary artery blood flow. In our study; the neutrophil / lymphocyte ratio in patients who were admitted to the cardiology clinic with the diagnosis of acute coronary syndrome after applying to the Emergency Department of Yeditepe University Kozyatağı Hospital; Coronary artery involvement, age, presence of ral, short-term prognosis, in-hospital adverse event, heart failure development rate and diagnostic value in NSTEMI and STEMI patients were evaluated. Materials and Methods For this purpose; after the approval of the local ethics committee and the written consents of the patients were obtained; 95 patients who were admitted to the cardiology clinic after emergency room between 01/01/2018 and 01/01/2020 with the diagnosis of acute coronary syndrome registered in the Hospital Operating System were included in the study retrospectively. Patients with a diagnosis other than acute coronary syndrome and whose files could not be accessed were excluded from the study. The patients included in the study were examined in terms of age, gender, presentation complaints, history, presence of ral, WBC, neutrophil, lymphocyte count, neutrophil / lymphocyte ratio, troponin, diagnosis, coronary angiography results, in-hospital adverse events and outcomes. Results 95 patients hospitalized from the Emergency Department to the cardiology clinic were examined. 74.7% (n = 71) of these patients were male and 25.3% (n = 24) were female. The mean age of all patients was 60.99 ± 11.37 years. The patients most frequently applied to the emergency room with the complaint of chest pain (84.2% (n = 80)). 38.9% (n = 37) of the patients were diagnosed with NSTEMI and 61.1% (n = 58) of them were diagnosed with STEMI. In the study, a statistically significant difference was found between in-hospital adverse events, post-infarction heart failure and poor prognosis according to NLR levels; these rates were found to be significantly higher in cases with an NLR measurement of 2.4 and above. Conclusıon Neutrophil / lymphocyte ratio has been associated with coronary collateral development, infarct area involved during acute coronary syndrome, in-hospital mortality, and coronary angiography results in the literature. Our study results show that NLR can be used as a predictor of short-term prognosis, in-hospital adverse events and development of heart failure.
