Evaluation of clinical efficiency of injectable platelet rich fibrine application in pained temporomandibular joint disorders

Abstract (EN)

Objective: The evaluation of the effectiveness of liquid platelet-rich fibrin administration in addition to the arthrosynthesis procedure to improve clinical symptoms in patients with painful temporomandibular discomfort in groups 3-4 and 5 according to Wilkes classification and with indication of arthrosynthesis. Materials and Methods: Thirty-six patients (17 females, 19 males) aged between 18 and 64 years and classified in groups 3,4 and 5 according to Wilke's classification were included in this study. Only 18 patients underwent arthrosynthesis (control group), and 18 patients underwent injectable platelet-rich fibrin injection in addition to arthrosynthesis (experimental group). Stratified randomization was performed in the groups. For this reason, Wilke's 3, Wilke's 4 and Wilke's 5 subgroups, consisting of 6 patients, 3 old and 3 young, were formed in each group. Visual analog scale (VAS), maximum mouth opening (MMO), lateral movement capacity and Helkimo Clinical Dysfunction Index measurements were performed at baseline, 10th day, 30th day and 3th month. Results: VAS, MMO, lateral movement and Helkimo Clinical Dysfunction Index measurements improved in both groups. In the comparison between the groups, it was found that the improvement in experimental group patients was significantly higher than the improvement in control group. In addition, the effect of liquid platelet-rich fibrin application on VAS and Helkimo Clinical Dysfunction scores was found to be significantly higher in Wilke's 3 group than Wilke's 4 and Wilke's 5 groups. Conclusion: In the treatment of patients with painful temporomandibular disorder, fluid platelet-rich fibrin injection in addition to arthrosynthesis procedure improved clinical symptoms. In addition, this improvement is higher in Wilke's 3 patients than in other patients.
