Abstract (EN)

Intensive care units are sections which are hospitalized of patients who have serious illness and frequently applied invasive practices. In intensive care units, the reasons of high rate of infection are frequently performed invasive practices and a positive correlation between rate of equipment utilizations and hospital infections.The purpose of this study is to determine the observing rate of ESBL positivity by year in our hospital and help for being taken necessary precautions. Antibiotic sensitivity was determined by using Kirby Bauer disc diffusion technique, and on the basis of CLSI criteria The double disk synergy test was applied for understanding the positivity of ESBL.In studying period for 5 years, 629 bacterial species were isolated from blood, trakeal aspirat, urine, wound, sputum, catheter and catheter tip cultures in the intensive care units. The rate of the species was 15.2 %, 28.5%, 23.7%, 24.1%, 1.7%, 4.8% and 2% respectively. The rates of samples by service were 35.1% anesthesia, 23.5% internal medicine, 10.8% cardiovascular, 2.5% brain surgery, 16.7% general surgery, 10.2% thoracic surgery, 1% child service.The distribution of ESBL by year were 17% in 2004, 21.8% in 2005, 27.3% in 2006, 18% in 2007 and 16.5% in 2008 in our study (for 629 bacterial species in total). The distribution according to factors were also 13.3% for E.coli, 1.5% for Klebsiella, 0.4% for Enterobacter and 3.8% for Pseudomonas respectively.In our study; proportional increase in 2006 has been concluded due to maintenance of the hospital in that year. The reason of increases in 2007 and 2008, the number of beds have tripled in our hospital. To reduce ESBL rates in intensive care units and to can be controlled, it has been concluded that hospital infection control committee should work more effectively and control periods should be intensified.Key words: ESBL, resistance of bacteria, antibiogram, intensive care unit.
