The effect of acute allerjic reactions on diastolic heart fonctions

Diastolic heart failure is observing heart failure symptoms, despite preserved systolic functions. Although its prognosis is better than systolic dysfunction, it is a known cause of mortality and morbidity. We think that active cytokines occuring in allergic reactions cause impairment of cardiac diastolic functions with both direct toxic effect and coronary spasm. We aimed to investigate the effects of acute allergy on cardiac diastolic functions.Randomly selected 50 patients who admitted to Department of Emergency Medicine, Faculty of Medicine at Firat University between May 2010 and December 2010, were complaining for rash, itching and diagnosed as allergy based on clinical and laboratory findings were included in our study as an allergy group. 30 healthy volunteers with excluded allergy based on clinical and laboratory data included in the study as a control group, and total 80 patients were randomly selected.Patients? routine biochemical parameters, serotonin and histamine levels were measured for a diagnosis of allergy. E and A ratios, DT, IVRT, lateral mitral annulus tissue Doppler velocities, color M-mode flow progression velocities were evaluated with Echocardiography.When basal systolic functions, diameters of cavities, wall thicknesses and biochemical parameters were compared, there was no difference between two groups. There were a significant difference between color M mode flow progression velocities, E ratios, E/A ratios and mitral lateral annulus tissue Doppler velocites measured with ecocardiography at 0-day and 5 th day in allergy group (Vp 0-day- 45.10 ± 11.03 cm/sec, Vp 5-day- 51.38 ± 6.86 cm/sec, E 0-day- 3.31 ± 0.88 m/sec, E 5-day- 3.12 ± 0.88 m/sec, E/A 0-day- 1.76 ± 0.56, E/A 5-day- 1.55 ± 0.49, Sm 0-day- 0.04 ± 0.02 m/sec, Sm 5-day- 0.05 ± 0.01 m/sec, Em 0-day- 0.11 ± 0.46 m/sec, Em 5-day- 0.10 ± 0.03 m/sec, p<0.05).We have seen deterioration of diastolic parameters after acute allergic reaction. Acute allergic reaction leading to development of diastolic heart failure may be cause of mortality and morbidity.
