The comparison of application effects of propofol and magnesium on the inflammatory responce caused by tourniquet

Abstract (EN)
After tourniquet applications for some patients with orthopedic lower extremity interventions, I/R injury may be observed in these patients. The aim of this study is to comparision between effects of propofol at sedation dose and magnesium application on inflammatory responce under general anesthesia.45 patients with tourniquet for one-sided lower extremity intervention and ages between 18-60 years, and ASA between I-II were included to this study. Under general anestethesia, no additional drug was given in control group. Propofol was given to propofol group before and after induction at 0.2 mg/kg and 2 mg/kg/h dose, respectively. Before induction of magnesium group, magnesium was given intravenously at 30 mg/kg dose and continued for 10 mg/kg/hour. Levels of nitric oxide, superoxide dismutase and interleukin 1 beta and 10 were measured before induction, after intubation, 5 minutes before tourniquet relexation, 5 and 20 minutes after tourniquet relaxation.It was observed that according to basal values, levels of malondialdehide increased at all periods in control group, significantly began to decrease in propofol group, and also significantly decreased at 5th minute of ischemia and reperfusion in magnesium group. Levels of nitric oxide significantly increased in control group (p<0.05), but also increased in other groups. However levels of superoxide dismutase were increased at all periods in all groups, according to basal values (p>0.05). Levels of IL 1 beta significantly decreased in magnesium gruops, but also decreased in other groups. According to basal values, levels of IL 10 significantly increased in control groups, and increased in magnesium groups (p>0.05), but decreased in propofol groups (p>0.05). No difference was determined at the same period in all parametres of groups.We concluded that for general anestesia in tourniquet with additional magnesium application or propofol at sedation dose for interventions antioxidant magnesium application decreased additionally inflammatory response within groups according to basal values but this effect did not show significant difference according to control group.
