Evaluation of Glasgow Coma Score, optic nerve sheath diameter and hemodynamic properties of patients who developed and did not develop vasospasm after coil embolization treatment in patients with aneurysmatic subarachnoid hemorrhage

Abstract (EN)

Aim: Due to the wide variety of etiological causes that may cause changes in consciousness, the clinician must resort to auxiliary examinations. The aim of this study is to evaluate the daily state of consciousness, hemodynamics, and its relationship with vasospasm in patients with aneurysmatic subarachnoid hemorrhage who underwent coil embolization for treatment, by measuring the optic nerve sheath diameter with the help of ultrasonography for 14 days in the postoperative intensive care unit and ward. Material and Method: Our study is a prospective observational study conducted in the Anesthesia Intensive Care Unit of Karadeniz Technical University Faculty of Medicine between December 2022 and June 2023. Patients between the ages of 18 and 85 who were interned in intensive care were included in our study. Demographic characteristics of the patients included in the study, optic nerve sheath diameters evaluated by bedside ultrasound, systolic and diastolic arterial pressures, and heart rate measurements were recorded. The Glasgow Coma Scale was used as the change of consciousness scale. Results: No significant difference was found between patients in terms of demographic data. In patients who had aneurysmatic subarachnoid hemorrhage with underwent coil embolization and developed vasospasm, optic nerve sheath diameter measurements were found to be statistically significantly higher between the10th and 14th days, compared to patients who did not develop vasospasm (p<0.05). In patients who developed vasospasm after coil embolization, Glasgow Coma Score values between the 6th and 14th days were found to be significantly lower than in patients who did not develop vasospasm (p<0.05). Conclusion: It is thought that vasospasm that may occur in the postoperative period can be predicted by measuring the optic nerve sheath diameter for 14 days in patients with aneurysmatic subarachnoid hemorrhage who underwent coil embolization, and this may have a positive contribution to reducing morbidity.
