Prognostic factors in patients with sudden hearing loss and our results of therapy

Abstract (EN)

Sudden hearing loss (SHL) is a sensorional hearing loss that develops suddenly in three days or less than this, keeps minimum three frequency and minimum 30 db and more. For SHL etiology, many factors have been accused but an accurate reason has not been found yet. There is no common protocol applied for its theraphy. In this study, the results of therapy of phenomenons with sudden hearing loss and prognostic factors that affect these results are analysed95 phenomenons and 97 ears which applied Gaziantep University with sudden hearing loss diagnosis between the years 1992-2008 were included in the research. The detailed stories of all patients were acquired. Rutin ear nose and throat (ENT) examinations and detailed laboratory analysis were made. Odiologic extrapolations of all phenomenons were made. Radiologically cranial magnetic resonance visualization (MRG) and computer tomography (CT) were made. In therapy, 1mg\kg steroid, diatrizoate (ürographyin), diüretic ve vazodilatator medicines were given to all phenomenons. Within one week, asiclovir was added to the theraphies of applied phenomenons. In this research, the story information of phenomenon, clinic findings, the degree of hearing loss and odiologic configurations, phenomenon?s times of beginning to theraphy, diabet,vertigo, tinnitus and the presence of upper respiration tract infection and the effects of these factors on prognosis were analysed.In this research, 57 of phenomenon were male and 38 of them were female. Age gap was 7-40. (average 47.71) In 7 (%7.2) phenomenon weak, in 18 (%18.6) phenomenon middle, in 29 (%29.9) phenomenon high and in 43 (%44.3) phenomenon very high sudden hearing loss were determined. As for the time of starting to therapy, in patients who started therapy early statically meaningful results were taken in terms of replying to the theraphy. In phenomenons with SHL, statically meaningful results were gained in terms of intensity of the hearing loss and replying to therapy . In the SHL phenomenons with no diabet or vertigo statically more meaningful results were taken in terms of replying the therapy as compared with the SHL phenomenon with diabet and vertigo. In terms of tinnitus, odiologic configuration, and presence of upper respiration tract infection, no meaningful results were acquired as prognostic factors in the phenomenon with SHL.The phenomenon who started therapy early, with no diabet or vertigo, with middle or weak degree SHL, better results were taken to the threapy. Diabet, vertigo, the time of starting therapy and degree of SHL are meaninful prognostic factors in phenomenon with SHL.KEYWORDS: Sudden hearing loss, Etiology, Prognostic factor, Therapy
