The effect of serum mir-210 and mir-214 levels on diagnosis and treatment in ankylosing spondylitis patients

Abstract (EN)

The aim of this study was to investigate the effect of microRNA-210 (miR-210) and microRNA-214 (miR-214) on bone loss and disease in ankylosing spondylitis (AS) patients receiving anti-Tumor Necrosis Factor (Anti-TNF) and Non-Steroidal Anti-Inflammatory Drug (NSAII) treatment. In this study, 34 AS patients aged 18-65 years (17 Anti-TNF drug users and 17 NSAI drug users) and 16 healthy volunteers with a matched mean age and gender distribution who had been diagnosed for at least 2 years and who applied to the Rheumatology outpatient clinic of Gaziantep University, Department of Physical Medicine and Rehabilitation were included. Detailed clinical evaluations were performed in patients and volunteers. Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) and Bath Ankylosing Spondylitis Functional Index (BASFI) and VAS (Visual Analog Scale) scores were recorded to evaluate disease activity in the patient group. Erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) levels were measured in serum samples of patients and healthy volunteers. IL-17A, CTXI and PINP levels were measured in serum samples using ELISA method. MicroRNA-210 (miR-210) and microRNA-214 (miR-214) levels were measured using quantitative polymerase chain reaction (qPCR) method. MiR-210 values were similar in the "Anti-TNF and NSAII groups" and statistically significantly higher than the "Control" group (p<0.05). A statistically significant difference was observed in miR-214, IL-17A, CTXI values "according to groups". Accordingly, all groups were found to be different from each other and were ranked as "NSAII, Anti-TNF and Control" groups according to high values (p<0.05). A statistically significant difference was observed in PINP value "According to the groups" Accordingly; "Anti-TNF and Control groups" were similar and "NSAII group" had a lower value (p<0.05). A statistically significant positive correlation was found between "VAS" and "BASDAI and BASFI values", between "BASFI" and "BASDAI", between "CTXI" and "miR-214", between "PINP" and "IL-17A" in AS patients receiving anti-TNF treatment (p<0.05). A statistically significant positive correlation was found between "VAS" and "BASDAI, miR-210, IL-17A values", between "BASFI" and "miR-210", between "CRP" and "ESR", between "IL-17A" and "PINP", between "CTXI" and "miR-214" in patients receiving NSAII treatment (p<0.05). A statistically significant positive correlation was found between "CRP" and "ESR" values in the healthy control group (p<0.05). MiR-210, miR-214, IL-17A, PINP and CTXI values were significantly higher in the AS patient group than in the healthy control group (p<0.05). A statistically significant positive correlation was found between "CRP" and "ESR value", between "BASDAI" and "BASFI, VAS, IL-17A and PINP values", between "BASFI" and "VAS and miR-210 values", between "VAS value" and "IL-17A and CTXI values", between "miR-210 value" and "miR-214 value", between "miR-214 value" and "CTXI value", between "IL-17A value" and "PINP value" in AS patient group (p<0.05). It was observed that miR-210 value had a statistically significant discrimination with Sensitivity (79.4%) and Specificity (75.0%) values (p<0.05). It was observed that miR-214 had a statistically significant discrimination with Sensitivity (76.5%) and Specificity (75.0%) values (p<0.05). The results of the study revealed the relationship between miR-210 and miR-214 levels with inflammation and clinical parameters and also showed that anti-TNF treatment was more effective than other treatments on inflammation, bone destruction and clinical course. As a result of the results obtained, it is thought that miR-210 and miR-214 levels will be meaningful in the diagnosis of the disease, the relationship of miR-210 and miR-214 levels with IL-17A, PINP and CTXI levels may be helpful in monitoring the course of the disease and the results will contribute to the development of new pharmacological treatments.
