Evaluation of consultation results of patients using antithrombotic and anticoagulant drugs before oral surgery

Abstract (EN)

The aim of our study is to present the data regarding the consultations written before oral surgery of patients using anticoagulant and antithrombotic drugs for various reasons and to examine the risk status of these individuals during oral surgery. The age and gender distribution of the patients sent for consultation and the medical arrangement protocols recommended by the medical doctor to the patients were examined in proportion. This work; The physicians working in the unit before the treatment of 300 patients who applied to Zonguldak Bülent Ecevit University Faculty of Dentistry, Oral and Maxillofacial Surgery Department for oral surgical treatment between 15/01/2022 and 15/06/2022, were under antithrombotic or anticoagulant treatment due to cardiovascular diseases. The study was conducted by prospectively examining the consultation forms written by and answered by the cardiology.Patients over the age of 18 were included in the study. The ages of the patients ranged from 23 to 85 years, with a mean of 62.05±10.62 years. 39% (n=117) of the patients were female and 61% (n=183) were male. 17.3% (n=52) of the patients received anticoagulant, 46.7% (n=140) antithrombotic, 12.3% (n=37) new generation anticoagulant, and 23.7% (n=71) combined treatment. While the regimen was changed in 18.7% (n=56) of the patients, the regimen was interrupted in 54% (n=162) and continued in 27.3% (n=82). The most commonly used drugs are ASA and Clopidogrel. The most frequently interrupted drugs were ASA with 38.3% (n=62) and Clopidogrel with 37% (n=60). 57.7% of the consulted patients (183 patients) are in the high-risk group and it is a correct surgical decision to consult these patients. The high-risk group has a relationship with age, gender and the drug used, and it is important to identify these patients. Patients using new generation anticoagulants or ASA or Clopidogrel alone have a low risk of bleeding and do not need to interrupt the pre-procedural regimen. In patients using antithrombotic and anticoagulant drugs, the regulation or evaluation of drugs used before oral surgery is done with a guideline created based on scientific evidence, and it is thought-provoking that these guides are constantly updated and although they are easy to access, there are still non-standard applications. It is extremely important to follow the guidelines prepared by the authorities in order to reduce the risks of possible bleeding complications and to standardize the treatments.
