The results of surgical treatment of acetabulum fracture

Abstract (EN)
Objective: In this study, the clinical and radiological results of the patients with acetabular fractures we treated with the open reduction and internal fixation technique were evaluated. Patient life quality effects was surveyed with Short Form-36. Material and method: Between January 2003 and November 2012 in Dicle University Faculty of Medicine Department of Orthopaedics and Traumatology, 37 acetabulum fracture of 37 patients treated with the open reduction and internal fixation technique. Patients operated average in 3,8 days (1-10). Thirty of the patients were male, seven of were female and the average age was 36,4 (17-58). All fractures were close. Twenty two (%59,5) of fractures were on the right hip and 15 (%40,5) were on the left hip. Travma mechanism was; in 19 patients traffic accident (in vehicle),5 patients traffic accident (out vehicle), 8 patients fall from high,3 patients motorcycle accident and 2 patients simple fall.16 of patients had additional orthopaedic injury. Pre- op acetabular fracture graphics were classified according to Judeth and Letournel. Accordingly 22 (%59,5) of fractures were simple type and 15 (%40,5) were complex type. Totally in 27 (%73) patients traumatic hip dislocation were determined, 20 (%54,1) were posterior, 5 (%13,5) were central and 2 (%5,4) were anterior. Dislocations were reducted with close reduction technique in emergency service. Findings: Patients average follow up time was 49,8 (12-117) months. In 32 (%86,5) patients reduction was anatomic (0-1 mm), 3 (%8,1) patients succesful (2-3 mm) and 2 (%5,4) patients bad (>3 mm). According to Matta's radiologic evaluation criteria; %78,3 excellent and good, %10,8 average and %10,8 bad results had seen. According to clinic evaluation criteria (Merle d'Aubigne and Postel) totally %67,5 excellent, %24,3 average and %8,1 bad clinic results has achieved. When clinical evaluation (Merle d'Aubigne and Postel) results gets closer to the excellent, a similar increase in the results of seven sub parameters (except Mental Health parameter) of Short Form-36 v and when radiological evaluation results gets closer to the excellent, a similar increase in the results of eight sub parameters of Short Form-36 had seen. Result: In treatment of acetabular fractures with reduction in proper time, proper incision choosing and sufficient surgical experience satisfacting results has achieved. To evaluate the life quality, Short Form-36 is an effective survey that corelated with clinical and radiological results. Keywords:Acetabulum,surgery,fracture,internal/fixation.
