4. SUMMARYAs in all parts of the world, the cost resulting from asthma is very high in our country.Such things as people?s applying to emergency service and their being nursed in hospitals fortreatment, that result from our not having been able to take the asthma under control, are themain factors increasing the asthma. Because of the high cost of the drugs in developingcountry, the is a need for this study in our country.Asthma is one of the chronic diseases common among the kids and adults. Treatmentfailure cases have been observed in some patient groups in spite of putting the regulationsdetermined by the consensus reports into practice. Upper airway related pathology such assinus pathology and nasal polip have been diagnosed in some patients of this group andduring the treatment of these disorders the asthma has also been treated. This makes anecessary searching of the following pathology in asthma patients and especially in resistantasthma cases.OBJECTIVESinus pathology influences the severity and frequency of the disease in asthmapatients. In this research, we aimed at contributing to the precautions reducing the frequencylevel of the disease in asthma patients with sinus pathology and its occurence frequency inasthma patients of our region. We aimed to evaluate this by examining the frequency and typeof sinusnasal pathology based on clinic complaints, anterior rinoscopic symptoms, nasalendoscopic symptoms, allergic test (nasal smear, ECP-eozinofilic katronic protein) total andspecific IgE) and on the sinus graphic and the computer tomographic pictures of paranasal.METHODFifty patients, 28 women and 22 men, being diagnosed with asthma and underobservation have been included in this research. We have put these patients, being in differentages between 15 and 60 and having had sinus-nasal complaints, into two groups as atopic andnonatopic with the results of allergic parameters by searching their anterior rinoscopicsymptoms, nasal endoscopic symptoms, Water?s graphics and the anatomic variations inparanasal tomography and by looking at the levels of nasal smear, ECP, Serum TOTAL andspecific IgE.SYMPTOMSIn the % 72 of the patients included in our research group sneezing, in % 70 of themcoughing, in % 70 of them the pain in threat, in % 68 of them headache, in % 12 of them thetasting abnormality, in % 4 of them anosmi have been obsorved. The percent of nasal pouringhas been about 64 and that of postnasal pouring has been 46. The observed ratio of other7symptoms haven?t been found meaningful in terms of statics except the nasal suffocation ratiobetween sexes and the occurence of sore throat with atopic and nonatopic patients.Polip has been observed in the % 14.63 of the nonatopic patients and in the % 22.22 ofatopic patients ın % 50 of our patients pathology has been found through Water?s graphic, butthis has been 60 through BNS PT. In our research, sinus pathology mostly covers the regionsof maksilla.RESULTWe accept the diagnosis of sinonasal pathologies is very important because there issupplied a current improvement at the patient?s life standarts if the sinonasal pathologies atthe asthma patients are treated. We believe the tomography of sinus has to be taken in order todetermine possible sinus pathologies especially for the asthma factors which are hardlycontrolled.Endoscopy and BT are complementary investigations and by endoscopy the nature ofthe pathology and by tomography its spreading can be detected and evaluated. In this study,we agree the evaluation of sinonasal patologies by computer tomography is the rightestapproach, although the nasal endoscopy is generally a good test in order to determine middleconca pathologies and nasal cavity structures. As a result, examination of the nose cavity viaendoscopic method is more valvable than classical anterior rinoscopy in the evaluation ofnasal secretions, middle meatus and lateral nasal wall in the policlinic conditions withoutapplying topical anesthesia.We thing that for the asthma patients suspected for allergical respect, investigating theallergical respect parameters with rutin investigations will contribute the treatment planningand etyopatogenesise of the disease. So, the treatment for the patients with who aredetermined with allergy, will increase the success rate by normal treatment with the treatmentfor allergy.At the result of our study, we concluded the increasing number of the anatomicalvariations of the patient?s, does not incraease the avarage opasiphity scorin of theirs at thatside. In this study, we didn?t determine an increase on the total variation numbers of thepatient?s and we saw they were accordance with the literature.8
