The relationship between left atrial thrombus and spontaneous echo contrast with beta-fibrinogen 455 G/A polymorphism and glycoprotein 3a HPA1-b polymorphism in the cases with atrial fibrillation

Abstract (EN)

Aim and rationale: The detection of genetical defects about clotting factors,which lead to thrombus formation in atrial fibrillation can help to determine the risk of thromboembolism and mechanism of thrombus formation.In our study; we aim to investigate the relationship between left atrial thrombus and spontaneous echo contrast with the polymorphisms of beta fibrinogen 455 G/A and glycoprotein IIIa PlA1/A2 in which the predisposition to arterial thrombus was observed.Method: Totaly 47 patients in whom 24 patients with thrombus in TEE, and 23 patients without (control group) were included in the study. Patients were evaluated in terms of mitral regurgitation grade and presence of SEC (Spontaneous Echo Contrast) apart from the presence of left atrial thrombus. After inclusion; DNA analysis was conducted to determine the beta-fibrinogen 455 G/A polymorphism and glycoprotein IIIa PlA1/A2 polymorphism in 3-5 cc blood which taken from the peripheral venous route.Results: In the patients with left atrial thrombus; mitral regurgitation score was detected less which adjacent to significance (p=0.08) and the SEC was detected significantly more (p=0.03). Beta-fibrinogen 455 G/A polymorphism was detected about % 44.4 in the group with left atrial thrombus and/or dense SEC, and about % 10 in the group without left atrial thrombus and/or dense SEC (p=0.01). Glycoprotein IIIa PlA1/A2 polymorphism was detected about % 22.2 in the group with left atrial thrombus and/or dense SEC, and about % 25 in the group without left atrial thrombus and/or dense SEC (p=0.82).Conclusion: Beta-fibrinogen 455 G/A polymorphism was detected significantly more in the patients with dense SEC and/or thrombus in left atrium as for glycoprotein IIIa PlA1/A2 polymorphism was detected similarly with control group. Inversely proportional relationship was established between left atrial thrombus and mitral regurgitation grade which is adjacent to statistical significance. Lineer relationship was established between presence of thrombus and SEC density in left atrium.
