Investigation of relationship between the biochemical features of csf in different sex and spinal blockade

Abstract (EN)

Spinal blockage is one of the oldest and the most effective regional anaesthetic technique. Level of the spinal anaesthesia is affected by the anaesthetic agent and the differences in the application methods. Besides, personal factors such as age of the patient, height, intraabdominal pressure, anatomical structure and the features of CSF (composition, circulation, volume, pressure and dencity of CSF) is the determinating factor for the level of spinal anaesthesia. In this study, we investigated how the differences in the biochemical parameters of CSF between male and female affect the level of spinal anaesthesia. 120 patients who scheduled for various surgical operations by spinal anaesthesia in Akdeniz University Medical Faculty were in rolled the study and formed two group as male and female. After the transfer to the operating room, vital parameters, sensorial and motor blockade of the both groups were followed in specified periods. CSF and blood samples taken before the operation were evaluated biochemically and compared. It was seen that, vital parameters in both study group were significantly affected by spinal anaesthesia. During the first 30 minutes of the operations, sensorial and motor blockade was evaluated in every 5 minutes. Sensorial blockade level was found significantly higher in male group and the onset time of motor blockage was found significantly shorter in the female group. According to the biochemical assessment, there was no significant differences in the blood parameters between male and female whereas albumin, glucose, Ca+2, Mg+2, protein and urea levels were significantly higher in the male CSF. CSF parameters spinal bockade correlation was evaluated. There was a (+) correlation between sensorial blockade and pH, dencity, glucose and urea levels in male group and between albumin in female group. The correlation between spinal blockade and level of Na+ and urea was (-) in females. In conclusion; in this study, it is shown that there were some differences in some BOS parameters between male and female and this differences might affect the spinal anaesthesia. But further studies are needed in this area. In addition, it should consider that maximal block levels were higher in male group. Because of this, it is recommended to be careful during spinal anaesthesia in the point of blockade levels in males
