Abstract (EN)

In our study, the kidney renal artery resistive index values and renal renal artery resistive index values by doppler USG in the early postoperative period (24-48 hours) of patients who underwent kidney transplantation from living donors and cadavers due to Chronic Kidney Failure at Gaziantep University Medical Faculty Hospital Kidney Transplant Center between January 2018 and December 2021. by looking at the number of graft kidney vessels. The effect of the number of renal graft arteries on the graft functions and the effects of which findings, whether the resistive index has a predictive place, by performing statistical analysis with the recipient, donor information and operation data obtained from the hospital database and control examinations. In addition, we aimed to retrospectively examine the effect of the number of arteries on the resistive index, how the number of arteries and the resistive index are related to the long-term graft functions. In the study, the etiology of chronic renal failure, duration of dialysis, additional disease, urinary status, compliance, radiological findings, age, gender, body mass index, postoperative 0. 1. 3. 7. day and 1. 3. 6. month creatinine values, postoperative Data such as need for dialysis, reoperation status and graft loss were reviewed retrospectively. Complications such as acute rejection, bleeding, graft loss, reoperation, which developed during the 6-month period after renal transplantation, as well as the relationship between renal graft function markers such as creatinine level and dialysis need, with renal graft resistive index and number of arteries were investigated. While the cause of renal failure in 124 (41.6%) patients in the patient group examined in our study was unknown, the cause of kidney failure was HT in 70 (23.5%) patients, DM in 17 (6%) patients, and HT and DM simultaneously in 15 (5%) patients. detected. A total of 298 recipient patients were included in the study. 195 (65.5%) of the patients were male and 103 (34.5%) were female. The youngest of the recipients was 6 years old, the oldest was 74 years old, and the mean age was 37.84±15.16 years. The mean BMI value of the patients was 24.73±5.45. It was found that the mean value of the postoperative Resistive Index did not show a statistically significant difference with respect to graft loss (p>0.05). It was found that the mean Postoperative Resistive index value of those with 1 artery number was statistically significantly lower than those with 2 arteries (p=0.041, p=0.028). It was observed that the mean postoperative renal artery Resistive index value did not show a statistically significant difference according to whether the donor was alive or cadaver (p>0.05). A positive, very weak, statistically significant correlation was found between age and Resistive index value (r=0.139, p=0.016). In our patient group, 263 had 1 artery (88.26%), 33 had 2 arteries (11.07%) and 2 had 3 arteries (0.67%) 13 of those with graft loss had 1 artery, 3' 2 arteries were seen. No graft loss was observed in patients with 3 arteries. It was found that there was no statistically significant relationship between graft loss and number of arteries (p>0.05). In addition, when the relationship between the number of arteries of the renal graft and creatine was examined, it was observed that the presence of multiple arteries in the long term did not have any negative effect on creatinine values. As a result, it was thought that the presence of multiple arteries in donors did not prevent kidney transplantation, and the long-term results of graft functions were similar to kidney transplantations with single arteries, and there was no difference in terms of Resistive index values.
