Evaluating the relationship between primary nodal generalized osteoartrhitis with tissue antigens HLA-A and HLA-B in Bolu region.

Abstract (EN)

Nodal generalized osteoartrhitis (GOA) is known as subunit of osteoarthitis (OA) and is characterized with OA in 3 or more joints and Heberden and Bouchard nodul formation in the interphalangeal joints. Exact etiopathogenesis is still unknown. Important risk factors includes age, genetics and sex. Occupation, race, geography and climate are among the other potential but unclear risk factors. GOA occurs frequently in postmenopausal women. Because of its close relationship with HLA A1 and HLA B8 antigens GOA is suggested as a autoimmun disease. Medical therapy, physical therapy, orthesis and surgery are therapeutical options.In this study, we aimed to evaluate the relationship between the patients diagnosed as nodal GOA, in Bolu, according to the criterias of the American College of Rhemautology, with tissue antigens HLA A1 and HLA B8. 46 GOA patients with differantial diagnosis (Group I) and 60 patients without GOA (Group II) were included the study. Blood samples were collected from both groups. Patients were choosen in same ratio of sex and age in both groups. In Group I age, sex, body mass index and family history datas were collected to evaluate the etiopathogenesis. Radiographies were staged according to the Kellgren-Lawrence scoring system.In Group I 4 patients (% 8.96) were male, 42 patients (% 91.3) were female and in Group II 5 patient (% 8.3) were male, 55 patients (% 91.6) were female. Mean age in Group I was 64.74±8.46 and was 62.32±6.8 in Group II. Mean BMI in Group I was 28.86±4.7 and was 30.82±4.38 in Group II. There was a stastistically significant difference (p<0.05) between two groups mean BMI. We didn?t find any relation between obesity and nodal GOA. % 54.3 of the patients? hand nodal OA radiographies were diagnosed as stage 4. There wasn?t a statistically significant relationship between radiographic nodal hand OA staging and BMI (p=0.369).We didn?t find any relation between Nodal GOA and HLA-A1 an HLA-B8 haplotypes (p>0.05). In our study, conversly to the other studies, we found a statistically significant relation between nodal GOA and HLA-A2 and HLA-B38 haplotypes (p<0.05). HLA-A29 was not found in any person of the group I where it was found % 11.7 in Group II. The difference between two groups was statistically significant (p=0.040).Despite limited number of cases, we thought that, this study provides adequate data to show relationship between different genetic factors and environment-ethnic structures.
